Responsibilities*

Venous Health Program (0.5 FTE)

  • Participate in comprehensive venous disease management in collaboration with vascular surgery, vascular medicine, and interventional radiology.

  • Partner with the emergency department, inpatient units, and outpatient teams to develop safe, effective, and efficient care coordination for this patient population. Provide education for patients, families, and healthcare professionals, and contribute to the development, implementation, evaluation, and outcome monitoring of clinical care.

  • Outpatient management of patients with venous disease in collaboration with vascular surgery, vascular medicine, interventional radiology, and other relevant departments.

  • Develop and maintain comprehensive tracking and follow-up procedures for patients within the venous health program.

  • Serve as a local, state, and national liaison and consultant for venous management programs, and continue to explore novel approaches to healthcare delivery for this patient population.

  • Incorporate research findings into clinical practice and contribute to quality improvement activities related to the venous health program.

Vascular Surgery (0.5 FTE)

  • Provide care to patients and families in the Vascular Surgery Clinic at the Cardiovascular Center.

  • Serve as a clinical resource and consultant to patients, families, UMHS providers, and referring providers in the management of complex vascular problems.

  • Interview patients to obtain detailed medical histories, perform examinations, and present pertinent data to medical staff.

  • Collect and assist in interpreting diagnostic and laboratory test results, radiographic procedures, and clinical evaluations.

  • Order tests and prescribe medications under physician supervision.

  • Collaborate with medical staff to deliver coordinated care for patients and families.

  • Make internal and external referrals as needed for comprehensive patient care.

  • Provide appropriate written and electronic documentation regarding patient status, evaluations, and treatment plans.

  • Work with the multidisciplinary team to develop and implement patient education and clinical pathways, particularly for chronic and complex wound management.

  • Participate in department meetings to improve clinic processes and workflows.

  • Engage in continuing education and professional development to maintain competency and stay current on the care of complex and chronic wound patients.

Organizational Relationships:

  • Supervision of professional nursing practice is received from the APP manager.

  • Supervision for delegated medical functions is received from designated physicians.

  • Maintain collaborative and consultative relationships with other care providers, disciplines, and departments.

Program Management:

  • Develop and promote venous management services through cost-effective and innovative strategies.

  • Determine and track outcome metrics for both the venous health program and vascular surgery services; analyze data for continuous improvement.

  • Coordinate patient flow between vascular surgery, vascular medicine, interventional radiology, and other services as appropriate.

Education and Professional Development:

  • Participate in educating healthcare professionals and students through mentoring, in-services, seminars, and formal presentations.

  • Engage in activities that advance nursing practice, such as professional associations, publications, and clinical research.

  • Maintain and develop clinical knowledge and skills through continuing education, coursework, and professional literature.

  • Contribute to self-evaluation and create plans for ongoing professional development and learning.

Research:

  • Assist with ongoing and proposed research initiatives related to vascular and venous health care. Contribute to new study development and the incorporation of research into clinical practice.

Required qualifications must be met by the candidate in order to be interviewed and considered for the position. Required Qualifications*

For Nurse Practitioners (NP):

  • Current licensure to practice as a Registered Nurse in the State of Michigan.

  • Current specialty certification as a Nurse Practitioner in Michigan.

  • Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AG-PCNP).

  • Graduate of an accredited Nurse Practitioner program.

  • Certification recognized by the National Council of State Boards of Nursing (NCSBN).

  • Current ACLS certification.

  • Current DEA registration.

  • A minimum of one (1) year of clinical experience in the last 5 years working with vascular surgery, surgical, or cardiology patient populations.

For Physician Assistants (PA):

  • Satisfactory completion of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies (CAHEA or CAAHEP).

  • Certification by the National Commission on Certification of Physician Assistants (NCCPA).

  • Current license to practice as a Physician Assistant in Michigan.

  • Current ACLS certification.

  • Current DEA registration.

  • State of Michigan Controlled Substance License required.

  • A minimum of one (1) year of clinical experience in the last 5 years working with vascular surgery, surgical, or cardiology patient populations.

Desired Qualifications*

  • Experience managing patients with venous disease.

  • Experience in the development of evidence-based clinical protocols.

  • Experience with professional presentations, group education, and healthcare marketing.

  • Participation in clinical research.

  • 2-5 years of recent experience as an NP/PA caring for vascular or general surgery patients.

  • At least 1-2 years of experience managing patients in a clinic setting, preferably in a cardiovascular setting.

  • Experience treating patients with conditions like peripheral artery disease, venous disease, wounds, and aneurysms.

  • Demonstrated proficiency in reviewing diagnostic studies such as CT scans, angiograms, ultrasounds, venograms, and MRIs.

  • Proven ability to work collaboratively with multidisciplinary teams, manage time effectively, and solve problems efficiently.

Work Schedule

Hours/Week: 40 hours

Shift/Days/Hours: Day shift; specific days and hours to be determined. May include evenings and weekends.

Location: Cardiovascular Center Clinics or other locations as needed.
