Job Description

We are seeking Physical Therapists to join our home health physical therapy agency. In this role, you will be responsible for advising patients and their families about in-home treatment options, providing educational information about injury prevention, ergonomics, and ways to promote physical health. You will also be expected to consult and collaborate with other healthcare professionals, document patient care history, and comply with rules, regulations, and procedures.

Your duties will include administering treatments involving the application of physical agents, administering massage, soft tissue mobilization, joint mobilization, and conferring with physicians and other practitioners to obtain additional patient information, suggest revisions in the treatment program, and integrate physical therapy treatment with other aspects of the patient's healthcare.

You will also assist in providing therapy services defined in treatment plans in residential settings, work with physicians, case managers, and adjustors, evaluate patients determining the level of patient's physical and psychosocial functioning, develop and revise the plan of care in consultation with the physician and other care-team members, diagnose movement dysfunction and develop a treatment plan, teach patients how to properly use therapeutic exercise techniques, and provide stimulation or massage to promote healing.

The ideal candidate will have certification as a Certified Physical Therapist (CPT), and a state license to practice physical therapy. Home health experience is preferred but not required. This is a full-time position. The successful candidate will have a strong commitment to working safe, demonstrated ability to work in a team environment, be highly motivated, well organized, and committed to high-quality patient care.
