Position Summary:

The Heritage Community Clinic Registered Nurse (RN) promotes the improvement of health for underserved individuals and communities across Southeast Ohio through direct patient care and screenings, education, and outreach. This position will provide nursing support for clinics serving uninsured and underinsured individuals in a 21+ county region through the Heritage Community Clinic and fleet of mobile health units. This position is responsible for the scheduling, promotion, and day-to-day operations of clinical outreach services in the Heritage College’s Community Health Programs.

Primary Responsibilities:

  1. Direct Patient Care
  • Serves as a member of the Community Health Programs ( CHP ) and Heritage Community Clinic team; coordinates, assists with oversight, and implements community health outreach clinics throughout Southeast Ohio for underserved, uninsured/underinsured, and/or low-income patients

  • Provides nursing management and care for Heritage Community Clinic in line with Heritage Community Clinic policies and procedures and ensures compliance with Standards of Care within the RN scope of practice

  • Responsible for providing high quality, age-appropriate, and culturally competent care in a compassionate, non-biased manner, ensuring adherence to the highest ethical standards

  • Provide follow-up and referrals as needed; responsible for answering questions and being the referral source for physicians, community health providers, and the public

  • Connect patients with Community Health Programs ( CHP ) and Area Health Education Center ( AHEC ) staff, as well as community partners, to address both clinical and social determinants of health

  1. Clinic Coordination and Outreach
  • Responsible for all program scheduling and coordination as well as other duties including attending meetings, advertising program services and performing other administrative tasks related to clinic operations

  • Coordinating scheduling processes for providers and patients in a 21+ county area

  • Communicate available services to the Southeast Ohio community through various media channels, community organizations, and more

  1. Charting and Documentation
  • Document patient encounters in an appropriate and timely manner in the electronic medical record

  • Ensures compliance with program guidelines through accurate record keeping and reporting, including but not limited to timely Salesforce data entry; analyze program data to ensure that clinics are meeting established goals

  • Assist with the management of databases to identify and collect information for reporting and evaluating programs

  1. Patient and Community Education
  • Provide age-appropriate, culturally competent patient education on issues ranging from immunizations to chronic disease management

  • Precept medical, nursing, and health professions students

  • Maintain current knowledge of patient care and community resources, attending trainings and community meetings as warranted

  • Serve as an instructor of American Heart Association Basic Life Support courses and other health related activities for students, community members, health professionals, and AmeriCorps members

  • Utilize nursing knowledge and engage community groups such as K-12 schools, college students, social services agencies, and others through health-focused presentations

Minimum Qualifications:

  • Associate’s degree (RN) with five years of relevant experience OR Bachelor’s degree ( BSN ) with three years relevant experience.

  • Valid RN license in the state of Ohio.

  • Current American Heart Association Basic Life Support Healthcare Provider certification.

  • Must be willing to obtain American Heart Association Basic Life Support Healthcare Provider. Instructor certification.

  • Valid Ohio driver’s license.

Preferred Qualifications:

  • Bachelor’s degree in Nursing ( BSN ).

  • Three years of relevant clinical experience and two years of experience working with an Electronic Medical Record ( EMR ) system.

  • Demonstrates excellent customer service skills and the ability to engage effectively with the public.

Advertised Salary:

The approved hiring range for this position is $51,281 to $67,947. Offers outside of this range of subject to additional approvals. The final offer will be commensurate with the candidate’s education and experience in relation to the minimum qualifications for the position Pay is accompanied by our generous university benefits such as free tuition for self and dependents, a comprehensive insurance package including but not limited to:
