Shift: 7am – 5:30pm

Location: Emory Heart Transplant Center, Emory University Hospital, 1364 Clifton Rd, Atlanta,

Georgia, 30322

We are seeking a capable Clinical Transplant Coordinator whoprovides leadership in collaboration with the health care team to define and integrate practice models for designated transplant patient populations throughout the continuum of care.

  • Serves as an expert of knowledge and nursing practice with the transplant population. Functions as educator, consultant, researcher, manager and collaborator.

  • Provides transplant program information during negotiation of managed care transplant contracts.

  • Manages patient populations to preserve efficient, cost effective, quality care to both the patients and Emory Hospitals.

  • Identifies targets, assesses the situational variables and provides the appropriate situational leadership style to effect appropriate outcomes. Develops policies and procedures for the delivery of care to the transplant population.

  • Administers direct patient care to meet the special needs of each age specific patient population. Coordinates transplants during the work day and after hours. Under the direction of the physician, may assume 24-hour telephone management of pre- and post-transplant outpatients.

  • Conducts comprehensive health assessments and demonstrates a high level of autonomy and expert skill in assessing the complex responses of individuals and families to actual and potential health problems.

  • Assesses, analyzes and synthesizes clinical findings to result in optimum goal achievement for patients in a cost-effective manner.

  • Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. Develops and maintains patient management systems (i.e.: transplant database, patient records, UNOS and CTRD). Participates in data collection for department research projects.

MINIMUM QUALIFICATIONS:

  • A master's degree in nursing preferred and a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.

  • Four years of recent clinical nursing experience as a Registered Nurse which includes at least two years in a specialty area, one of which is ICU preferred.

  • BLS certifications required.
