Description Lutheran Social Services is currently seeking a Care Coordinator with a substance use recovery focus in a new program serving Sauk County. The Care Coordinator will work with Sauk County residents who are incarcerated to prepare for release from county jail or state prison and smoothly transition back to the community with the appropriate support services. . This is a Full Time position and benefits eligible. The Care Coordinator will be housed at the county jail to facilitate access to inmates. They will complete a risk assessment at booking (at the jail or court) and identify potential eligibility for intervention programs. The availability of re-entry services will be discussed with incarcerated individuals if they meet criteria. The Coordinator will monitor the jail population and identify Sauk County residents who will be incarcerated a minimum of 30 days and are scheduled to release within three months. As this is a new position and program, the role will also require some program development of tasks and responsibilities in collaboration with program leadership. Under direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals. Performs other duties as required.
