The Miracles of a Second Chance, Inc. behavioral health paraprofessional, BHPP, will provide behavioral\ health services to residents of Miracles Of A Second Chance in accordance with treatment plans, policies and\ procedures, philosophy, and objectives. The care provided will focus on the mental, spiritual, emotional,\ nutritional, and physical needs of each resident to ensure that quality of life remains optimal for each\ resident. Supervision will be provided by a State Board of Arizona licensed behavioral health\ professional or a board certified professional under A.R.S. Title 32 or a behavioral health professional\ pursuant to R9-10-703.\ Duties:\ 1. Report to duty promptly at the appointed times as reflected by the posted schedule.\ 2. At all times maintain residents rights, dignity and privacy while attending to the needs of the\ individual.\ 3. Assist residents with activities of daily living (grooming, dressing, undressing, eating, exercise,\ and personal hygiene) and document in the residents clinical record.\ 4. Check each residents clothing on a daily basis to ensure that there are no buttons missing,\ tears or improper fitting garments.\ 5. Understand the treatment plan for each resident and follow it, unless the resident refuses\ treatment, in which case documentation must be made in the clinical record that treatment\ was refused.\ 6. Assess the residents emotional well-being, physical well-being and mental status on an hourly\ basis and report any changes noted to the Program Director immediately.\ 7. Assist with the self-administering of medications in accordance with policies and procedures.\ 8. Follow the posted menu without deviation and offer snacks as posted.\ 9. Offer eight (8) ounces of fluid every hour during the day to ensure adequate hydration.\ 10. Assess the amount of fluid and portion of food eaten at each meal and document in the\ clinical record. Notify the Miracles Of A Second Chance nurse should any resident refuse fluids, food, or\ medication during a twenty-four (24) hour period.\ 11. Call 911 for all medical emergencies as referenced in the medical Emergencies Procedures\ Manual. Give CPR, if necessary, until medical personnel arrive.\ 12. Notify the Miracles Of A Second Chance nurse and complete incident report for any injuries, medical\ emergencies, resident complaints, and stolen property belonging to a resident.\ 13. Do all assigned household chores on each shift.\ 14. All other duties as assigned.
