POSITION SUMMARY STATEMENT: All colleague tasks and responsibilities support the mission, vision, and values of HealthPartners and abide by all HealthPartners policies and procedures. Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation, and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and HealthPartners and Employer specific protocols and policy. Provide and manage direct nursing care to patients with a specific disease or health condition requiring Anticoagulant medication(s). Autonomously, and in collaboration with managing clinicians, apply nursing judgment and standing orders to provide assessment, education, planning, and implementation of ambulatory anticoagulation care within the scope of Registered Nurse practice including delegation and supervision of licensed and unlicensed staff. Act as a resource for other staff. MINIMUM QUALIFICATIONS: Education, Experience or Equivalent Combination: Completion of an accredited nursing program Bachelor Science - Nursing (BSN) required Associate Degree in Nursing will be considered if currently enrolled in and/or willing to obtain Bachelor of Science in Nursing degree within three years of hire Minimum of two years recent nursing experience in adult ambulatory care and one year of recent experience in an acute care setting or associated specialty: Anticoagulation/Thrombosis Care, Cardiology, Hematology/Oncology Application of nursing theory, regulations, and practices Licensure/ Registration/ Certification: Current Minnesota RN license Wisconsin RN license within two months of hire if applicable Current American Heart Association BLS certification Knowledge, Skills, and Abilities: Excellent demonstrated verbal and written English communication skills Excellent customer service skills; tact and diplomacy in interpersonal interactions with patients/ families/clinic staff members. Demonstration of compassion and caring in all interactions Understanding of patient education needs, able to effectively share information with patients, family members and clinic staff as needed Ability to learn, retain information regarding patient care procedures. Ability to plan, prioritize and complete delegated tasks, and remain calm in stressful situations Strong keyboarding skills Experience using electronic medical records Familiarity with Microsoft Office suite (Outlook, Teams, Word/Excel) Telephone/headset use Fax/Copier/Printer PREFERRED QUALIFICATIONS: Education, Experience or Equivalent Combination: Anticoagulation Therapy Management course completion Experience with identifying problems and physical and psychosocial assessment of patients receiving anticoagulation therapy Licensure/ Registration/ Certification: Knowledge, Skills, and Abilities: Essential Duties: % Of Time (minimum 5% increments) 1. Assessment/Nursing Diagnosis Effectively assesses care needs of patients and care givers, prioritizes patient needs utilizing the nursing process; determines nursing diagnosis focused on care and identifies patient outcomes; develops patient care plan; implements and continuously evaluates the care plan; ensures patients are discharged with resolved educational needs Collects and prioritizes data in a systematic and ongoing process that involves the patient, care giver, other health care providers and environment as appropriate Uses appropriate evidence-based assessment techniques and instruments in data collection Analyzes and interprets assessment data to determine nursing diagnoses Formulates and revises nursing diagnoses that reflect the current patient status and communicates this with the patient, care giver and other colleagues within the health care team 2. Planning of Nursing Process Develops plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles Establishes a plan that provides for conti uity of care Develops an individualized education plan for patients and care givers based on identified learning needs Uses evidence-based research data blended with caring and compassion, to develop the plan of care and interventions 3. Implementation/Evaluation of Nursing Practice Coordinates, delivers, and delegates nursing care that is safe, ethical, and culturally competent based on patient assessment/standards of care in a timely manner Utilizes systematic and ongoing assessment data to revise diagnoses, outcomes and the plan of care Involves the patient, caregiver, and health care team members in the evaluation process when appropriate Utilizes evidence-based interventions and treatments specific to the diagnoses as appropriate Initiates and implements plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles Provides comprehensive, individualized healthcare teaching, using the teach back method, to patients and caregivers Actively participates in creating and implementing improvements at the point of care to achieve clinical quality, patient experience, nursing engagement, efficiency, and effective outcomes Evaluates the patient's progress toward attainment of the outcomes and collaborates with members of the care team when outcomes are not as expected Evaluates and documents the patient's/care givers' understanding of and response to the plan of care 4. Documentation Documents patient care and education accurately, timely, and completely in the EHR (electronic health record) Reassesses, revises, and documents the plan of care based on the patient's response to treatment and nursing interventions on an ongoing basis 5. Patient Care Responsibilities Acts as a patient advocate and provides emotional, intellectual, and psychological support to patients and their families. Demonstrate clinical competence using the nursing process to provide care to patients and their families in-person or telephonically, regardless of worksite location Implement established Nursing Policies, Procedures, Standing Orders, and Clinician Orders using available medical data, patient interview and assessment Delegates appropriately and holds self and others accountable for safe delivery of the delegated care. Interviews patients and/or family members to assess the patient's educational needs, current knowledge of condition, and degree of understanding and motivation to follow anticoagulation guidelines. Develops and implements education plans for individuals taking anticoagulation therapy and performs ongoing evaluation of the effectiveness of these. Notifies patients of test results and determines follow-up appointments as appropriate. Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation. Listens and evaluates patient concerns and redirects to primary care team as indicated. Document completely and timely, to keep accurate record regarding clinical practice; forwards charts and completes billing appropriately 6.
