SCHEDULE:: 40 hours, Monday - Friday, 8:00 a.m. - 4.30 p.m.; with rotating on call coverage
Pays $29.55 per hour DOE

JOB DESCRIPTION:
Summary of the Position:
The Case Manager Supervisor will be responsible for the specific caseload of tenants at the New Chardon residence under the Behavioral Health Division of Pine Street Inn, a program that provides permanent housing to formerly homeless women using a Housing First/Stabilization model. The program serves women from diverse backgrounds, circumstances, and age groups. The mission is to provide trauma-informed, gender sensitive and gender specific support services that meet the special needs so they can heal and have time to heal, prepare to reintegrate into the community, and break the cycle of homelessness. Tenants will be assessed for their actual level of functioning in various spheres upon entering the program and supported in individual plans to improve and enhance this level. The Case Manager Supervisor is responsible implementation of effective best practices (Motivational Interviewing, Comprehensive Case Management, Harm Reduction, and Trauma - Informed Care) and ensuring the provision of high quality case management supports on a 24/7 basis. The Case Manager Supervisor will directly supervise and provide support to a Case Manager and an Activities Coordinator. The incumbent will be a great team player who is also self-motivated, with solid communications skills, flexibility and creative problem solving.

Education/Training:
Required:
Bachelor-s Degree in Social Work or other related field such as Psychology, counseling, psychosocial rehabilitation or equivalent relevant years of experience.
Preferred:
A valid Massachusetts Driver-s License in good standing
Master-s Degree in Social Work or other related field such as counseling, Psychology, psychosocial rehabilitation
Advanced training in psychosocial rehabilitation and substance abuse
Bi-lingual - Spanish/English
Knowledge/Experience:
Required:
Minimum of at least (2) years- experience working as a Lead Case Manager and/or coaching and mentoring and/or providing supervision to staff in the human services and related fields
Minimum of three (3) years prior work experience working with homeless individuals suffering from complex Trauma, SUD diagnosed, disabilities in an housing settings
Strong clinical background and familiarity with providing Trauma Informed Care and Harm Reduction Treatments to dual diagnosis populations
Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment
Experience conducting psychosocial assessments
Experience with case management and implementing treatment plans
Knowledge of treatments for chronic mental illness, including medications
Previous experience working with dually diagnosed individuals
Experience assisting people with ADL skills
Experience working with vendors and services providers within the public mental health, housing and SUD systems
Previous experience in providing support for individuals experiencing homelessness and mental health issues into housing
Preferred:
Previous experience providing Clinical supervision to staff in the human services or related fields
Previous experience working with a Housing First/Stabilization model
Familiarity with the Metropolitan Boston public and private mental health system
Experience working with individuals experiencing Long term mental health issues

Essential Functions Include But Are Not Limited To:
Carry a caseload of 10-20 tenants and meet regularly with each individual tenant to address barriers and provide support services to address needs.
Work with tenants to create client-centered, strengths- based individual treatment plans and periodically evaluated and edited as necessary for best practice and in accordance with PSI program standards
Perform ongoing clinical assessments, updating documentation and maintaining and updating tenant charts.
Have daily discussion with program staff to promote program integrity and to proactively identify and develop plan/intervention to address any issues
Educate, coach, and reinforce with tenants activities of daily living (ADL) skills, including, but not limited to personal hygiene, budgeting, cooking and leisure time activities. Assist tenants in shopping, laundry, and other ADL as needed or requested.
Meet regularly with each tenant to seek opportunities for ongoing psychosocial rehabilitation.
Work with the Program Manager to evaluate tenant progress and to create best practice intervention to support tenants.
Coordinate with other departments and programs within PSI, including but not limited to the Housing Stabilization Team, to facilitate successful case management outcomes.
Work cooperatively with other agencies to secure needed levels of support for individual tenants, advocate within the mental health system for timely and appropriate services for tenants as needed and requested.
Guide tenants in an intentional progression method towards increased autonomy, improved life skills, and enhanced quality of life.
Provide clinical and administrative supervision to the Case Manager and Activities Coordinator according to program policies. Coach and support staff to enhance supports to tenants in a strengths-based, person-centered model as they move towards increased autonomy, successful tenancy, and improved life skills, and enhanced quality of life.
Hire, evaluate, and provide training, and support to new staff, while providing a supportive and encouraging personal and professional growth for current team members.
Encourage tenant involvement in creating and enhancing as sense of connectedness and community, while fostering life satisfactions, accountability and responsibility.
Work with Program Manager to track program performance and program goals and outcomes.
Attend and facilitate team meetings with staff for treatment planning and activity planning
Support Program Manager to conduct the team meetings, case consultations, treatment planning, and activity planning in union with The Director, Housing Stabilization team, and other stakeholders.
Meet for regular supervision with the Program Manager.
Advocate within the mental health system for timely and appropriate services for tenants as needed and requested.
Accompany tenants to outside appointments as needed or requested.
Work together with the Activities Coordinator to assist tenants with improving their overall quality of life.
Maintain necessary records and statistics needed for program reports.
Participate in selection process for prospective tenants.
Network with other Housing First/Stabilization and residential programs in Boston to gain ideas and enhance program efficacy
Encourage and coach tenants on the process of being a responsible neighbors and build community awareness and involvement.
Develop treatment plans with the ultimate goal of assisting tenants move into appropriate permanent, affordable housing.
Perform other additional assignments or projects as requested/directed by supervisor.
