Job Description:

Plans, implements, renders and evaluates social work services in the home care setting using professionally approved assessment and therapeutic skills and modalities in accordance with the scope of practice for a Licensed Medical Social Worker in New York State.

Minimum or Preferred Qualifications:

Master’s Degree in Social Work. One (1) year experience providing patient/client counseling. Current licensure to practice as a Medical Social Worker in NYS. Valid NYS drivers license.

Salary Range:

$33.12-$39.12 per hour Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
