Summary:

The Coordinator of Panel Management utilizes patient data to conduct various outreach activities to close gaps in care and support patient health improvement. The coordinator engages in focused intervention activities such as, prospective panel management review to include pre-visit planning and retrospective panel management review to include outstanding/overdue report management; panel attribution; and alterations driven by improvement playbook implementation. The coordinator will engage in strategic gap closure efforts including, direct patient outreach, provider and clinical team outreach, and external health care provider outreach. This position reports to the Manager of Panel Management.

Responsibilities:

  • Performs prospective panel management review to include pre-visit planning activities; surfacing important information to close gaps in patient care.

  • Performs retrospective panel management review to include outstanding/overdue report management.

  • Performs panel attribution alterations and other activities associated with the implementation of the improvement playbook.

  • Performs strategic gap closure activities/functions as related to identified intervention and improvement projects.

  • Provides direct patient outreach for gaps in care, including preventive visits, screenings and chronic conditions.

  • Provides direct provider, clinical care team and community health provider outreach.

  • Manage and update patient information in the electronic health record.

  • Manage patient appointments, orders and referrals as within scope of outreach and gap closure functions.

  • Performs work according to established best practices, standard work and developed workflows.

  • Other duties as assigned.
