As a Case manager Personal Care Services you’ll contribute to our success in the following ways:

  • Obtains a medical history, particularly as it relates to the present condition, from the member and/or family member(s).

  • Evaluates the client’s condition and home situation to determine if other services will be required and the frequency of this service.

  • Surveys the client’s environment to determine what assistance will be available from family members and other informal supports in caring for the client.

  • Assesses the client’s family member(s), home situation, to determine if other services or health teaching will be required and the frequency of this services.

  • Interprets nursing and other services to clients and their families as a part of planning for care.

  • Develops the client’s service plan and coordinates the authorized services.

  • Monitors the effectiveness of the service plan per Company policy and State and Federal Program regulation.

  • Submits case management progress notes and other documents outlining the services rendered as indicated.

  • Submits a tally of visits made each day.

  • Participates in case conferences and staff development meetings.

  • Discusses with the supervisor problems concerning the clients and how they may best be handled.

  • Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement the client’s service plan.

What is Required?

  • High School Diploma/GED

  • 1+ year RN or LPN experience

  • Must have a dependable vehicle, valid driver’s license and current auto insurance in accordance with the state.

  • Proficiency with MS Office Suite

  • Previous experience with Workday is preferred

You will report to the Case Management Supervisor.
