La S ante Health Center is seeking a Care Coordinat or Manager to oversee all Care Coordinators. The ideal candidate will lead the coordination efforts to ensure seamless patient care delivery and effective communication between healthcare providers and patients. Additionally, they will develop and implement strategies to optimize care coordination processes and improve patient outcomes.
Responsibilities:
Manage care coordinators, ensuring smooth patient care and delivery
Periodically monitoring for quality and reporting to management
Communicating between staff, patients and family
Handling patient cases
Training staff and creating schedules
Developing patient goals and monitoring progress
Maintaining patient confidentiality and quality care
Qualifications:
Manag ement experience
Care Coordination experience preferred
Clinical experience preferred
