Description
Comunilifeis looking for an experienced Mental Health Counselor for its DHS transitional program in Brooklyn.The Mental Health Counselorprovides mental health related services for homeless clients with mental health and or substance abuse needs. Responsible for the timely completion of mental health assessment and collaborating in the development and implementation of service plan(s) for each client admitted to the program. This position is eligible to participate in Comunilife's generous benefits and paid time off package.
Shift:
(1) Sun-Thur 9AM-5PM
(1) Tues-Sat 9AM-5PM
Essential Job Duties:
Conduct mental health assessments and assist in the development and implementation of comprehensive service plans that identify mutually agreed upon long and short-term mental health related goals.
Provide mental health services and referrals; assistance with adherence to medication regiments; activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training; prevention with positives; and other necessary services to clients to increase their capacity for independent living. Also, monitor and ensure that clients are linked to needed services.
Provides, coordinates and collaborates with providers within and outside Comunilife.
Provide supportive counseling to clients and coordinates and collaborates with mental health providers to help clients to comply with treatment and medication regime.
Make home visits and maintains face to face contact as specified in the service plan(s)
Responsible to develop and implement weekly groups including the Wellness Self- Management model.
Responsible for the timely completion of all required documentation including, comprehensive assessments, reassessments, progress notes of all contacts with clients and/or collaterals.
Participate in case conferences, training, workshops and seminars as well as meetings with peers and supervisors as required.
Assist in the planning, organizing program recreational activities.
Participates in weekly supervision.
Qualifications
Education and Experience Requirement(s)
Masters Degree, in Mental Health Counseling or Psychology, Social Work or related health field. Prefer one (1) to three (3) years work experience in mental health field, providing on-going counseling to HIV/AIDS individuals and their families. Individual, family and group counseling experience required.
Preference given to persons with experience in Department of Homeless Services Programs, bi-lingual- Spanish.
