Principal Objective/Role Overview:

The Nurse Care Coordinator Supervisor works with Graham’s multidisciplinary teams, Graham parents, foster parents, and external providers to ensure that all children in Graham’s care receive the physical health assessments, services, and follow-up to ensure that they are healthy while at Graham and post-discharge. The Care Coordination Supervisor is responsible for overseeing all aspects of the Nurse Care Coordinator’s role.

Job Responsibilities:

  • Provides quality supervision to direct reports inclusive of:

  • Regular and continuous observation and evaluation of staff’s client face-to-face interactions, documentation, and everyday practice to ensure that staff is consistently attending to their responsibilities of Care Coordination.

  • Ensures that there is ongoing assessment of staff skill development and schedules/monitors staff training. Provides consistent feedback on performance and supports staff in areas of need.

  • Provides guidance and training to staff on assessment/tools (i.e. following required 29i medical assessments, HIV risk assessment, etc.).

  • Reviews tracking reports on new admissions, medical consents, well child visits, HIV risk assessments, medical treatment planning, documentation of activities and medical record maintenance.

  • Ensures that there is continuous assessment of functioning and that care plans are reflective of current needs.

  • Facilitates changes to care plans and reviews direct report care plans to ensure that they are thorough and ensure quality care.

  • Reviews and signs off medical forms (i.e. school/camp forms, work forms, sports participation, WIC forms, adoption medicals, Special/Exceptional rate requests, Social Security letters etc.)

  • Ensures completion of documents is timely and responsive to youth/family’s needs.

  • Partners meaningfully with service providers/foster parents/discharge resources:

  • Includes foster parents and parents as needed in collateral sessions to obtain pertinent information related to child/youth functioning as it relates to medical symptoms and service needs.

  • Discusses child medical issues with caregivers and effectively coaches caregivers on how to manage child/youth’s medical conditions.

  • Provides caregivers training aimed at educating and supporting caregivers in managing the clinical needs of children/youth in their care. Conducts caregiver’s training on medication monitoring and collaborates with CP to monitor medication tracking logs.

  • Participates in permanency FTCs and provides input related to medical/behavioral health needs, service planning for youth and families.

  • Ensures that appropriate medical/behavioral health referrals/services are in place prior to transition from child welfare system.

  • Performs continuous quality reviews of performance/regulatory requirements:

  • Competent and effective coordination of audits and other regulatory tasks.

  • Ensures that there is consistent review of medical documentation (i.e. Connections Netsmart, Office Practicum and other medical records).

  • Ensures that physical plant requirements as they pertain to safe, secure, confidential provision of treatment are maintained.

  • Insures sufficient nursing staff coverage hours.

    Other Position Requirements:

  • Willingness and ability to travel when needed throughout several Graham sites located in Brooklyn, The Bronx and Harlem

  • Position is hybrid

  • Comfortable with remote work and supervising remote staff

    Salary & Compensation:

  • Base Salary or Hourly Rate: $80,000.00/year

  • FLSAStatus: Exempt

Qualifications

Position Qualifications:

  • Registered Professional Nurse NYS License required.

  • A minimum of 2 years postgraduate experience and at least 2 years of supervisory experience.

  • Familiarity with NYS/NYC Foster Care systems and requirements.

  • Experience in Pediatric/Adolescent Health Care preferred.

  • Strong collaborative and organizational skills are needed.

  • Comfortable with electronic health record documentation
