Formerly known as the Community Action Network (CAN), the Healthy Start Community Consortium (HSCC) is a community coalition focused on improving birth outcomes and reducing infant mortality in the City of Camden. The HSC Consortium (HSCC Coordinator) oversees the creation, development, and implementation of the HSCC. Facilitates regular community meetings with consortium members to develop and implement an action plan for reducing disparities in perinatal health in the community of Camden, NJ. The HSCC Coordinator works with outreach case management staff to determine target audience and needs of CHS program participants and develops collaborative relationships with MCH providers, CBOs and local health department to ensure access to care and community-based resources.

Essential Functions:

  • Manages all activities of the HSCC and builds on existing CAN framework.

  • Serves as a liaison for CHS program and HSCC members.

  • Schedules, coordinates, and facilitates regular meetings and discussions with the HSCC members that supports systems change and improvement to reduce social determinants of health in the community we serve.

  • Supports HSCC members in the creation and implementation of an action plan for policy and systems level strategies to address disparities in perinatal health in the community of Camden City.

  • Coordinates cross-sector partnerships to ensure access to coordinated, comprehensive maternal, child, and family medical care, health, and parenting education; and community-based resources within Camden, NJ.

  • Monitors, analyzes, and interprets project area health status data with providers and consumers through the HSCC and appropriate subcommittees.

  • Participates in Communities of Practice with other Healthy Start projects and other technical assistant opportunities.

  • Provides community level trainings and updates on the HSCC planned activities.

  • Performs other duties as assigned.

Requirements

  • Associates Degree in Public Health, Social Work, or related field; Bachelor degree preferred

  • Camden City resident; current or former CHS participant desired

  • Minimum of three years’ experience in community relations and outreach

  • Knowledge of community resources in Camden City and general knowledge about MCH topics a plus

  • Strong meeting, planning, and facilitating skills

  • Excellent communication and public speaking skills

  • Demonstrated leadership skills

  • Good organizational skills: ability to prioritize workload requirements

  • Ability to work independently with minimal supervision

  • Ability to work well with a variety of individuals from diverse backgrounds

  • Knowledge of cultural diversity and cultural competency issues

  • Strong oral, written and interpersonal communication skills

  • Valid driver’s license, car, and current insurance policy

Physical Requirements:

  • Must be able to see to read documents and hear well enough to communicate with others

  • Must be able to write, type on a computer, and use a telephone

  • Must be able to operate a motor vehicle

  • Must be able to lift up to 20 pounds
