Under general direction, performs enhanced inmate services and release coordination for those with a Serious Mental Illness (SMI) determination, and inmates with exceptionally high needs due to mental health issues who may qualify for SMI determination.
Major Duties, Responsibilities
- Create, implement, and revise individualized case/release plans which include previous behavioral health and social determinants of health services, and historical criminal justice involvement for SMI inmates who qualify.
- Advocate for inmates with SMI determination, as well as those who are being evaluated for Rule 11 and Restoration to Competency.
- Work collaboratively with clinical and justice stakeholders to compile information to better identify treatment options and provide service linkage for SMI inmates upon release.
- Coordinate with the courts Mental Health Project Manager, and criminal justice partners to ensure all court appointments and obligations are met. Assist the courts in establishing release conditions that include inpatient and/or outpatient behavioral health services, as dictated by a treatment plan.
- Interface and coordinate with Detention personnel regarding the facilitation of services for SMI inmates.
- Perform other job-related duties as assigned.
