Description

REPORTS TO: Chief Operating Officer / LCSW

RATE OF PAY: $60,000 - $80,000 per year for full-time employment; part-time or per diem pay will be prorated based on scheduled hours

WORK SCHEDULE: Flexible within Monday - Friday daytime office hours; no evenings, weekends, or major holidays; no on-call; full-time, part-time, or per diem schedules available

POSITION SUMMARY:Under the clinical direction of a Licensed Clinical Social Worker (LCSW), responsible for providing individual therapy and psychological/social service to Health Center patients. This position also provides input and support for staff involved in the care of the patients.

PRIMARY RESPONSIBILITIES:

  • Provide psychosocial assessment, screening, counseling and crisis intervention services to health center patients using ethical and professional standards set forth by the National Association of Social Work.

  • Develop treatment goals and review/adjust plan as needed on a regular basis.

  • Work collaboratively with the multidisciplinary team to determine services that will be needed to assist patients and their families in reaching their goals of care and that promotes positive outcomes.

  • Prepare reports, progress notes and maintain patient records per agency policies and procedures.

  • Complete notes in a timely manner to ensure timely billing.

  • Work diligently to meet visit target numbers.

  • Perform other duties as requested by supervisor.

Requirements

  • Master’s Degree in Social Work

  • Registered NYS LMSW license

  • Experience with electronic medical record preferred.

  • Strong organizational and planning skills.

  • Ability to maintain good working relationships with others.

  • Exhibits a high level of professionalism and confidentiality in dealing with issues.

  • Sedentary work; ability to lift and carry up to 10 lbs.

  • Ability to sit for long periods of time.

  • Ability to tolerate exposure to computer monitor/screen.

  • Ability to tolerate use of computer keyboard.
