Pay Range $18.51-$23.60*

Quality Assurance - Transitional Care Coordinator – Full Time-Day Shift - 7a-3:30p– Req 7579

Patient Transition Coordinators work closely with RN Case Managers and Clinical Discharge Planners to ensure all transitional care processes are completed in an effective, efficient, accurate, and timely manner to assure compliance with regulations. Patient Transition Coordinators support RN Case Managers and Clinical Discharge Planners by working with partnering agencies to ensure the needs of each patient are fully met. Common duties include tracking processes and documentation from preadmission to post discharge, coordinating with hospital departments, and coordinating with outside agencies and insurance companies.

Patient Transition Coordinators support the RN Case Managers and Clinical Discharge Planners while they work closely with the patient and family, physician and other members of the healthcare team to move the patient from one phase of care to the next. They are not directly involved with the patient and family. However, they are responsible for all of the transitional services processes that occur in support of the patient and family from pre-admission through the time the patient is stable enough for discharge or transition planning.

Qualifications:

  • Minimum of 1-3 years’ experience in the medical profession

  • Must possess the ability to interact and communicate effectively with all levels of staff, patients, family members, visitors and physicians.

  • Effective oral and written communication skills.

  • Experience with policy and procedure writing.

  • Experience with state and/or federal survey agencies preferred.

  • Strong computer and database management skills.

  • Project coordination experience preferred.

  • Track all swing bed and in-patient admissions and discharges

  • Coordinate all swing bed screenings

  • Review appropriate length of stay and utilization of services

  • Work proactively to identify and decrease delays in transition

  • Work closely with partnering agencies and insurance companies under the direction of RN Case Managers and Clinical Discharge Planners

  • Receive, coordinate, and track all denials and appeals

  • Monitor outcome and perform variance analysis of quality data
