Greene County Medication for Opioid Use Disorder (MOUD) Patient Services & Linkage Coordinator in Catskill, New York

Greene County Public Health Job Announcement:MEDICATION FOR OPIOID USE DISORDER (MOUD) PATIENT SERVICES & LINKAGE COORDINATORHOURLY RATE: $27.62 (Grade 13, Step 0)This is a full-time, provisional appointment. Permanent appointment is contingent upon Civil Service examination.The MOUD Patient Services and Linkage Coordinator (Coordinator) will provide client focused substance use/addiction related services and will implement the philosophy of harm reduction using the appropriate tools and techniques. The position will be located primarily within the Greene County Family Planning Clinic (GCFP Clinic) and will report to the Public Health Director or the Family Planning Clinic Manager. The Coordinator will provide education, care coordination, and recovery planning to patients seeking inductions and on-going treatment with Suboxone and Sublocade, as well as who are seeking screening and referrals to treatment for HIV/Hepatitis C. The Coordinator also facilitates the clinic’s participation in the Bureau of Justice Assistance (BJA)-funded overdose prevention and patient linkage project. Does related work as required. FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:Good knowledge of client focused substance use/addiction related services; Good knowledge of harm reduction philosophy and the tools/techniques utilized in that philosophy; Good knowledge of recovery as it relates to health and well-being, peer support, and wellness and recovery planning; Good knowledge of software packages for word processing, spreadsheets, database, and video conferencing; Working knowledge of the various community resources and services available in the field of mental health; Ability to recognize recovery as the focus in all phases of addiction services, from community-based prevention to acute care to community integration; Ability to develop, implement, manage, monitor, and evaluate in partnership with people in recovery, their families, and advocates; Ability to provide support to individuals who may be only minimally receptive or in the very beginning of the change/recovery process; Ability to keep a record of client contacts as well as all other work-related activities; Ability to make informed decisions; Ability to work effectively with employees, staff of external agencies, and members of the public; Ability to embrace cultural diversity; Physical condition commensurate with the demands of the position; Good judgment, tact, and courtesy.MINIMUM QUALIFICATIONS: EITHER: A.Possession of a Bachelor’s degree in a Human Service or Health Science filed; ORB.Possession of an Associates degree in a Human Service or Health Science field and two (2) years of full-time paid work experience providing client focused substance use/addiction related services. ORC.Graduation from high school or possession of a high school equivalency diploma and four (4) years of full-time paid work experience providing client focused substance use/addiction related services. NOTE: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/US Secretary of Education. If your degree was awarded by an educational institution outside the United States and its territories, you must provide independent verification of equivalency. SPECIAL REQUIREMENTS: 1.Possession of a valid New York State driver’s license at time of appointment and this license must be maintained throughout employment. In addition, must have access to transportation to meet fieldwork requirements in a timely and efficient manner.
