The Care Coordinator functions as an integral part of the Clinical office staff and is responsible for coordinating the interdisciplinary care for a specific patient population by collaborating with administrative and clinical staff, physicians, and patients and their support groups. This position may incorporate the following roles, including but not limited to: direct patient care provider, advocate, educator, consultant, researcher, and quality manager. These duties shall be carried out in accordance with the Mission of University of Cincinnati Physicians, Inc. and the College of Medicine.
