Job Details
The Enrollment Coordinator is responsible for the engagement and enrollment of new Client in FLACRA’s HHUNY and CHHUNY Care Management Programs. This is done through outreach by performing activities that assist potential Clients in the community understand the benefits of and enroll into the Health Home Program. The Position requires skills in planning, organizing, and coordinating the delivery of Care Management services, as well as administrative support when needed. Strong skills and focus on engagement and motivational interviewing.
Duties
Manage assigned caseload of Outreach Clients by properly meeting requirements for contact and enrollment.
Verify and confirm a client’s eligibility for the Health Home Program by confirming Medicaid coverage, diagnoses, and risk factors.
Coordinate with FLACRA Providers and outside Community Agencies to gather needed documentation to qualify Clients into the Health Home Program.
Complete all enrollment documentation for a Client properly on paper and in Netsmart to successfully transition the Chart to a Care Manager once enrolled.
Document all efforts of enrollment into the Client’s Chart within Netsmart.
Build and use effective communications strategies among peers, medical staff, addiction and mental health providers and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology.
Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.
Participate actively in team meeting and supervision to enhance in professional development.
Maintain excellent record keeping.
Use of personal vehicle for work and potential transportation of clients
On-Call duties as assigned for nights and weekends.

Job Requirements:
Minimum Qualifications: BA, BS, QHP preferred. At least one year of work experience in the behavioral health field with knowledge of utilization management practices. Valid NYS Drivers' License.
