Position Type: Nursing Services/School Nurse Date Posted: 7/8/2024 Location: Cloquet Middle School (High-Needs School) LPN/Health Office Assistant (6.75 hours/day) Job Description: Position: LPN/Health Assistant Assist and work under the direction of a District Registered Nurse. Reports to: Building Principal/Licensed School Nurse Qualifications: Current LPN Licensure CPR/AED Licensure Be willing to accept nursing supervision Respect and protect the confidentiality of students, staff and families. Health Office/Sick Room - Direct Student Care: Medication Administration Proficient with Oral, Inhaler, Injection, Nebulizer, Rectal (Ativan) Monitor and recognize side effects of medication, communicate with RN Obtain proper and specific orders/signatures required both from parent and prescribing professional - NP, MD Contact parents as needed to request medications prescribed. Locate students if they do not show up to take medications. Medications must be administered daily as ordered. First Aid Cares Proficient and confident in your decisions and response to medical emergencies, recognizing emergencies such as seizure, asthma episode, low blood sugar, fainting, control of bleeding, swollen/deformed limb, food allergy, choking, chest pain/discomfort, head injury/neurological assessment. Confident in skills and ability to maintain calm demeanor in emergency or stressful situations. Medical Treatments Proficient with diabetic cares - recognizing hypo and hyperglycemia insulin administration, insulin pumps, glucose sensors, glucagon administration, carbohydrate counting and assisting with diet/intake management. Asthma care - knowledgeable with different types of inhalers and asthma controller medications, lung sounds, knowledgeable in safe range of oxygen saturation measure/readings. Treatment and management of seizure, knowledge of and how and when to appropriately respond. Wound care First Aid Food Allergies - management, action and emergency plans, recognition, how to treat - use of epinephrine auto-injector. Health Care Plan Coordination Assistance Work with RN to complete health assessment, Hearing and Vision, possibly Tympanometry, health review and parent interview. Collaboration with families of students who have had long term illness/absences, surgeries, accidents, sports injuries, other injuries, etc. Assist with Attendance team Daily attendance review, call and follow up Knowledge and competence with concussion s/s, ongoing consultation and communication with athletic trainers, teachers, counselors and other school staff. Medical Record and Immunization Compliance Maintain accurate and complete health records, and enter data into Electronic Health Records Ensure all students enrolled and new incoming students are up to date on all school age immunization requirements according to Minnesota School Immunization Law. Follow up with parents/guardians of students who are not in compliance. Complete Minnesota Department of Health annual report by December 1. Knowledge of & use of MIIC - Minnesota Immunization Information Connection Collect and Review Annual Health Information Summaries Compile list of students with health care needs which need to be communicated to staff. Send/Communicate above compiled list to staff along with health care plans of students with food allergies, seizure action plans, diabetics, asthma, other complex diagnosis. Assist with the completion of Care Plans for students with Complex Medical Diagnosis. Knowledgeable in Common Communicable/Infectious Diseases in school setting/public health. Follow protocol and procedure including prevention, tracking and follow-up with MDH. Accompany students with health related needs on Field Trips as necessary. Communicate and Present to teachers and paras during staff meetings the health needs of students, educate staff on various health related diagnosis such as informing of and teaching s/s of diabetic responses/epipen/seizure precautions/ hemophilia, and others de endent on student needs. Third Party Billing - enter/document direct nursing services cares into Sped forms site for those who qualify. Attend child study teams as requested. May be asked to monitor and track attendance as requested. Assist Special Education staff with health file review, parent interview/call, Hearing & Vision screening for special education evaluations. Complete Hearing & Vision Screenings and referrals as recommended/requested from parents or teachers. Consultation, Hearing & Vision Screening, Height/Weight for ECS program with Children in Li l Lumberjacks. . Possible need to assist in Special Education Rooms as LPN/Para. Optional: Summer Work with Special Education Extended School Year in July. Other duties as needed: Knowledge of COVID signs, symptoms and protocol. COVID contact tracing, may occur outside of school hours - requesting to return to Churchill on evenings or weekends to complete requirements according to Minnesota Department of Health protocol. Assist with management of the Early Childhood Screening health files. Enter EC Immunizations into Campus Program. Kindergarten Roundup - assist RN, coordinate powerpoint and presentation annually to incoming Kindergarten families. Main Office Reception Duties and Assistance as needed - Coordinate Medical emergency drills and health incidents within the building. Must demonstrate courteous and cooperative behavior when interacting with families, students, school staff and visitors. Maintain absolute confidentiality of work related information, records, and school occurrences. May be asked to work outside normal business hours and work extended hours to accomplish assigned requirements of the position with administrative directive.
