Prairie Ridge Health Diabetes Educator - 1.0 FTE in Columbus, Wisconsin

PRH is looking for a team member to join our Nutrition and Diabetes Services Department in the role of Inpatient Diabetes Educator. This position is a 1.0 FTE (40 hours per week) with hours of 8:00am-4:30pm, Monday-Friday. The position will be 90% outpatient and 10% inpatient practicing in both Columbus and Beaver Dam. Summary:

An Inpatient Diabetes Educator is responsible for providing consultation, screening, assessment, reassessment, and education for patients with a variety of medical conditions as a member of the interdisciplinary care team on the acute care units at Prairie Ridge Health (PRH).   They initiate and complete inpatient consults including order set implementation/management, medication management and other appropriate orders and interventions using Inpatient Glycemic Management Guidelines and provide real time and planned education and support of direct patient care providers.  The Inpatient Diabetes Educator provides care and recommendations for the glycemic management of high acuity inpatients, and therefore must stay up to date with glycemic management medications, devices (Continuous glucose monitors, pumps, etc.), and evidence based guidelines.  Participates in developing and updating procedures, protocols and departmental policies.  The Inpatient Diabetes Educator plans and carries out continuous quality improvement initiatives in line with the most recent literature, best practice guidelines and standards in the inpatient setting and related clinics.  Works closely with the Quality Director, Nurse Manager, and interdisciplinary care team to establish and maintain a high level of glycemic management on the inpatient units that bridges to the clinics and outpatient departments.  Maintains applicable order sets and policies/procedures on an annual basis in collaboration with appropriate medical staff and committees.  Collaborates with the SSM System Collaborative Glycemic Management Group, and other collaborative groups as needed/required.   Facilitates compliance with The Joint Commission, diabetes program recognizing body, state and other regulatory agencies related to nutrition and/or diabetes care.  Participates in quality initiatives.  Counsels and educates patients, family members, employees, medical staff and community members on nutrition, diabetes and wellness topics. Floats to provide coverage in all areas within professional scope of practice, including weekend and holiday call rotations (as appropriate).

An Outpatient Diabetes Educator is responsible for providing Medical Nutrition Therapy (MNT) and/or Diabetes Self-Management Education & Training (DSME/T) in the context of the recognized program, including nutrition and diabetes screening, assessments and reassessments of adult and geriatric patients with a variety of medical conditions as a member of the interdisciplinary care team.  Initiates and completes MNT and/or DSME for Outpatient services, using facility approved Standing Orders.  Counsels and educates patients, family members, employees, medical staff and community members on nutrition, diabetes and wellness topics. Develops, conducts and evaluates educational classes and materials.  Participates in developing and updating procedures, protocols and departmental policies.  Facilitates compliance with The Joint Commission, diabetes program recognizing body, state and other regulatory agencies related to nutrition and/or diabetes care.  Participates in the development, implementation and monitoring of quality initiatives.  Floats to provide coverage in all areas within professional scope of practice, including weekend and holiday call rotations (as appropriate).

EDUCATION REQUIREMENTS/LICENSURE/CERTIFICATION/REGISTRATION

  • Registered Dietitian Nutritionist 

  • Three years of experience with diabetes management strongly preferred

  • Fifteen credit hours of diabetes-related continuing education required within 90 days of employment; Certified Diabetes Care & Education Specialist (CDCES) strongly preferred

  • Certification in CPR

FTE: 1.0 (40 hours per week)

Shift: 8:00am-4:30pm Monday-Friday
