Work Type: Full Time
Hours Per Week: 40 Hour/Week
Work Schedule: 7:00 AM to 3:30 PM
Summary:
The Clinical Coordinator is an important part of the health care team. Utilizes knowledge of quality care standards to enhance patient experience and comply with health care standards. Maintenance of electronic systems that support quality reporting and population management. Works in conjunction with other health care professionals to provide quality team based care to the patients. Must be able to perform administrative duties in a the fast paced medical care environment. Must manifest a positive and caring attitude at all times and possess the flexibility to change as needed. The Clinical Coordinator reports to the Nurse Manager.
Responsibilities:
• Maintains the patient's chart and updates the registry as needed. • Performs patient call backs per provider’s instructions. • Performs accurate, legal and ethical documentation at all times. • Follows up as indicated on INRs, labs, referrals, and open orders to ensure patient compliance . • Works as part of a team dedicated to maximizing office visits and assuring quality care for the people we serve. • Treats all internal and external customers with respect and dignity. • Assists office practitioners as directed.
