Northeast PA Area Health Education Center Navigator in East Stroudsburg, Pennsylvania
Northeast Pennsylvania Area Health Education CenterĀ (NEPA AHEC) is the Operating Site for the National Health Corps (NHC) Community Health Fellowship (CHF) Program under the umbrella of AmeriCorps. The NHC CHF is a grass roots community health service program that trains local residents in Greater Philadelphia, Northeast Pennsylvania, Delaware and Centra California as community health workers, digital navigators, and/or medical interpreters who are committed to addressing the unmet health needs of underserved populations and developing grass roots community health leaders.
NHC CHF members participate in the PA AHEC state approved Community Health Worker training and complete 1700 hours of service during a 46-week service term with one of our host sites. Members receive a $15/hour living allowance during, medical, dental and vision coverage if needed, assistance with childcare if eligible, and a $7395 Educational Award at the completion of the service year that can be paid to existing student loans or used for future educational opportunities.
Maternal and Family Health Services (MFHS) is an innovative health and human service organization dedicated to meeting the needs of the community through information, education and quality care. MFHS meets the health and nutrition needs of Northeastern Pennsylvania's women, children, and families through information, education, and quality care. Our services address community-specific needs, enabling women and men to provide the bast for their families.
The NHC CHF member in the Resource Navigator position with Maternal and Family Health Services will assist in building connections with community resources including health and social services, assist in connecting MFHS clients and the community with needed services to address the social determinants of health, organize outreach efforts to ensure ongoing collaboration with community partners and referral sources, and coordinate cross collaboration between MFHS programs and other providers. The Resource Navigator will guide clients through the various health and social service system complexities, assist in enrolling clients in the programs offered by MFHS, and serve as the liaison between community resources, MFHS staff, and clients; build effective working relationships with clients; and educate and assist clients with navigating through the parameters set forth by community providers.
The hours are Monday through Friday 8:30 AM - 4:30 PM. Evening or weekend hours may be needed based on community events or clients' needs. Equal time off will be provided within the work week or hours worked and the member would have advance notice of schedule change. 1/2 an hour will be allocated for lunch which will not count towards member's service hours.
Major Duties and Responsibilities
Build strong professional relationships with clients to facilitate client engagement in all aspects of care; communicate information to clients to facilitate informed decision-making and connect families to needed services including providing assistance with completion of applications and follow-up documentation.
- Serve as an MFHS liaison between the community, providers, staff, and clients by working collaboratively, maintaining effective communication, and building positive relationships to eliminate barriers to care.
- Will provide direct engagement with existing community partners and develop new partnerships with providers across diverse fields (including health care, mental health/behavioral health, education, career services, criminal justice, children and youth, housing, food assistance, domestic violence prevention, and other social services), accumulating extensive knowledge of the services available through them.
- Will provide ongoing and consistent outreach to facilitate client access to community resources and generate and sustain a steady flow of referrals to MFHS programs.
- Facilitate and or schedule appointments to al MFHS Services, Community Services, and Referral resources as required.
