Under general supervision, supports the Clinical Pharmacy team’s performance with 5 star/HEDIS medication quality measures and care coordination/intake of patients with chronic disease management needs with the Clinical Pharmacy team.

TYPICAL PHYSICAL DEMANDS : While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to walk and sit and occasionally required to stand. The employee must regularly lift and /or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.

TYPICAL WORKING CONDITIONS : Work is performed in an office environment. Involves minimal contact with providers, employees, patients and the public. Although a safety first work environment, employees in the position have potential exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinic environment.

Location: 1580 Valley River Drive, Eugene, OR

If you reside in Eugene, OR, you will enjoy the flexibility of a hybrid-remote role as you take on some tough challenges. You will be required to work onsite 3 day per week.

Primary Responsibilities:

  • Receive and collate all quality lists from insurance plans for medication adherence and other medication quality measures. Participate in insurance provider quality meetings

  • Perform outreach (calls, portal messages) to patients, pharmacies, and insurance providers to include but not limited to education and medication list clarifications. Support the patient to ensure pick-up of their prescriptions and with any barriers to adherence

  • Document patient communication and outreach in the EHR which may include the use of portal messaging

  • Address incoming phone calls for the Clinical Pharmacy team which may include scheduling of patients and helping to troubleshoot medication access concerns

  • Perform administrative/clerical related duties, which may include scheduling of meetings and meeting minutes, as required by the Clinical Pharmacy team

  • Assist and provide coverage for the Pharmacy Program Coordination Specialist in helping patient’s access medications through medication patient assistance programs

  • Respond to questions regarding medication formulary alternatives. This will require knowledge of medication classifications

  • Perform other duties as assigned

Required Qualifications:

  • High School Diploma/GED (or higher)

  • 4+ years of experience in health care

  • 1+ years of experience as a pharmacy technician or medical assistant

  • 1+ years of knowledge of medical practice, procedures, and methods

Preferred Qualifications:

  • 3+ years of experience as a pharmacy technician or medical assistant

  • 1+ years of experience with Windows, Excel and Power Point

  • Understanding of health plan related operations

  • Knowledge of medical practice, procedures, and methods

  • Ability to establish priorities and coordinate work activities

  • Ability to identify problems and recommend solutions

Soft Skills :

  • Assist in maintaining department quality assurance. Ability to maintain quality control standards

  • Skill in establishing and maintaining effective working relationships with patients, medical staff and the public.Ability to communicate clearly and effectively with clinicians, patients and coworkers

  • Must be flexible and a quick learner. Ability to be highly organized, detail oriented and able to manage multiple projects at one time.Demonstrate initiative and ability to function independently
