JOB TITLE: Senior Services Coordinator
GENERAL JOB DESCRIPTION
Programs assigned to AAA Services Coordinator: 1. Each Coordinator is assigned Counites of Region 1 AAA set by the SUA of Colorado (Logan, Morgan, Phillips, Sedgwick, Washington, Yuma) 2. Each coordinator is required to acquire, maintain, and hold current certifications in the following programs under the AAA. A. Colorado Certified SHIP Coordinator (State Health Insurance Program) B. Colorado ADRC Coordination (Adult Disability Resource Center) The AAA Services coordinator maintains and provides services to residents under the Title III, Old Americans Act for the state of Colorado and the AAA of Region 1 in their prospective assigned county or counties. The AAA programs include fundraising, budgeting, and community outreach. Develops and implements programs that align with the organization's mission and support the organization's goals. Creates program materials and ensures that program achieves stated objectives and goals. Responsible for special events, and other duties that include and are not limited to publicizing the organization and its programs to the community. Work daily with other programs, reporting, data collection, and other duties assigned to assist the AAA Director, SHIP Coordinator, Programs Assistant, and the Nutrition Supervisor along with all other Service Coordinators in the field.
MAJOR DUTIES AND RESPONSIBILITIES Provide assistance to the Area Agency on Aging Director, and upon request, other organizations in developing and planning services for the elderl Responsible for Outreach and Information and Assistance program operation in all 6 counties. Responsible for the coordination of ADRC programs and services within the counties to ensure senior's needs are met and to eliminate duplication of services by other providers. Responsible for advocating for the rights of seniors locally, statewide and nationally. Responsible for maintaining records essential to the operation and justification of the programs. Assist in volunteer recruitment. Acts as a liaison to the County Councils on Aging, Interagency groups and at other meetings where the Area Agency on Aging input is required or reque Responsible for conducting in-home and on phone assessments to determine eligibility and need for Older American Act programs. Create and make all other reports, time sheets, expense sheets, schedules, in a timely manner. Assist AAA Director, Nutrition Supervisor, and the Programs Assistant, in-reporting duties, and data collection reports. Maintain the ADRC funding requirements, and data collection within all 6 counties. Have a working understanding of applicable Federal and State regulations. Interact with all clients or prospective clients in a professional and courteous manner. Interact with other professionals in a respectful and courteous manner. Be able to work without supervision. Attend meetings and trainings as required. Provide Information and Assistance to the communities in Northeastern Colorado concerning the AAA / ADRC / SHIP programs. Other duties that may be assigned or needed.
Full Time - 40 hours per week - F/T benefits offered. WAGE: $20/hour
QUALIFICATIONS
Any combination of experience and training that would likely provide the required knowledge and abilities. (All Employees must pass a Pre-Employment drug test)
Experience: 2-3 years of full-time work or equivalent part-time experience and knowledge of working with the Senior Population, the disabled, social work, case management, CNA, RN, Gerontology or other equivalent job experiences.
Training: Required: Completion of a secondary school curriculum resulting in a high school diploma. Preferred: Associates degree, and or 4-year degree preferred.
NOTE: If not already acquired by applicant, The AAA Services Coordinator will need to obtain their SHIP Counselor Certification within 6 mo. of employment and ma ntain all certifications and always be in good standing with the state of Colorado.
License: A Valid Driver's License Other: Knowledge of: Programs and services available in the area. Understanding the needs of older adults Objectively evaluate complaints and problem solve Organizational skills Good telephone skills Complete reports and send in on time.
