South Middlesex Opportunity Council Housing and Resource Navigator in Framingham, Massachusetts

SUMMARY

Work closely with area college staff, community partners and SMOC staff to identify immediate and short-term needs to support housing stability. Provide street outreach, assessment and 1:1 support to the homeless young adult and unaccompanied young adult population in the Metro-West area.

PRIMARY RESPONSIBILITIES

  • Triage all new referrals and walk-in youth in need

  • Complete an intake with each client and work with the client and case management team to develop and carry out a rehousing plan

  • Assess housing and supportive services needs to determine which short-term financial assistance programs may be available to rapidly re-house the individual.

  • Help clients to obtain all necessary documentation and complete paperwork for housing related applications.

  • Assist clients with completing housing applications and negotiating leases with landlords.

  • Develop connections with landlords with a focus on developing new relationships with new landlords, particularly low-barrier landlords, and maintain an active listing of landlords/properties.

  • Establish a thorough list of housing resources across different regions of the state where the families may have ties and supports in place.

  • Work collaboratively with collateral providers including the Department of Transitional Assistance (DTA), the Department of Housing and Community Development (DHCD), the Department of Children and Families (DCF), the Department of Youth Services (DYS), Early Intervention, Legal Services, Behavioral Health Services (BHS), etc. to ensure coordination of services

  • Participate as a member of the CGRC team; attend all staff meetings and other required meetings.

  • Maintain client files to include completed intakes, re-housing plans, referrals made, and services accessed.

  • Ensure the services are operated in compliance within all federal, state and local funding requirements, as well as, agency policies and procedures.

  • Be available to travel around the region to meet with providers, potential clients and potential landlords and to attend meetings.

  • Gather required data, keep accurate client records and provide regular written notes including client updates, demographics, barriers and new resources, etc.

  • Other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • Experience providing vocational and educational supports.

  • Assessment, advocacy and case management skills.

  • Flexible schedule required.

  • Bi-lingual preferred.

  • Reliable transportation.

  • Must have a valid Massachusetts Driver's license.

PHYSICAL REQUIREMENTS

  • Must be able to conduct street outreach which often may include long periods of walking and standing while speaking with clients.

WORKING CONDITIONS

Desk space is provided within the CGRC Staff Office and Community Satellite sites such as the homeless shelters and area colleges. Frequent travel throughout the Metro-West area is required. As part of the responsibilities of this position, the Housing and Resource Navigator will have direct contact with the young adult population. Dress is business casual. A successful background check is required.

Monday - Friday 9:00 am - 5:00 pm
