Weld County Government Wellness Program Coordinator - Area Agency on Aging in Greeley, Colorado

Compensation Range

$26.05 - $33.00

Job Description Summary

Maintains all wellness programs by recruiting, communicating, marketing, and networking with clients, volunteers and providers as well as developing and providing a variety of trainings, conferences, and support services. This position is in-person and occasional telework may be an option after at least one year of satisfactory employment.

Job Description Evidence Based Classes (70%)

Develops, implements, and participates in evidence-based health programs for the community's aging population including, but not limited to programs such as A Matter of Balance, Tai Chi: Moving for Better Balance, and the Chronic Disease Self-Management programs. Schedules classes or workshops in Community Partner facilities. Manages registration, participation levels and participant activities. Develops and maintains an active volunteer recruitment process. Ensures volunteers complete required paperwork including background checks and schedule for activities. Manages and guides volunteer activity and performance. Maintains timely, accurate and complete records of all activities related to clients served in a manner that meets federal, state and the Area Agency on Aging's requirements. Develops and maintains program policies, procedures, and standards. Community Outreach (25%)

Develops and implements a community outreach and education plan including volunteer recruitment, marketing materials to inform the community and the aging network regarding all programs. Networks with appropriate community agencies and professionals to facilitate service access for all special programs. Assists the senior community and their families with general information and assistance services. Participates in community information and education activities regarding available senior services, service providers and the service delivery system. Provides presentation and information, education and training for the Aging Network and community groups on services and benefits programs available through the Area Agency on Aging and other health and human services providers. Other Duties (5%)

Assists in writing articles for various marketing outlets such as AAA newsletters and newspaper articles. Follows policies and procedures, the Weld County Code of Conduct and adheres to strict confidentiality guidelines and appropriate release of information outlined in both. Required Qualifications Successful completion of a bachelor's degree OR Four (4) years full time work experience involving a high level of customer interaction of which, two (2) year must be in a human services environment, or a combination of education and relevant experience appropriate to the requirements of the position. Relevant experience is defined as: (a) Experience in one of the following areas: Experience in the field of community health, recreation, long-term care, home heal h care, or other adult or aging programs. Experience building relationships and networking with community agencies. Experience building relationships and working with volunteers. Experience educating or presenting information to the public. Accreditation by a certified health and fitness organization. Candidate must possess ability to keyboard at 35 words per minute. Candidate must possess a valid Colorado driver's license and liability Insurance by the employment start date and maintain for the duration of employment. Candidate must provide own transportation and be willing to travel throughout Weld County. Candidate must pass background checks prior to employment start date. Candidate must possess the ability to enter data into several different computer programs. Candidate must possess the ability to learn and apply new computer skills on a regular basis. Candidate must possess the knowledge of computer programs such as Microsoft
