Marshfield Medical Center-Dickinson Social Worker in Iron Mountain, Michigan
The Marshfield Medical Center-Dickinson seeks a full-time Social Worker *to join their team of healthcare professionals in *Iron Mountain, Michigan.

WHAT WE OFFER.
- $51,064 -- $79,476 annual salary based on experience.

- Full-time Monday-Friday position (36 hours per week) with start times of 7:30am or 8am.
WHAT YOU WILL DO.
- Provide direct social work services to patients and families, addressing social, environmental, financial, and emotional issues.
- Assist patients and families in coping with stress and anxiety related to medical conditions.
- Facilitate patient placement in nursing homes and home health, and seek options for financial aid, chore services, transportation, and nutrition programs.
- Develop and coordinate discharge plans, referring patients to appropriate social services and community resources.
- Consult with hospital staff, community agencies, and medical staff to resolve patient problems and interpret social/psychological, emotional and family issues.
- Coordinate the hospital swing bed program and monitor patient eligibility; inform physicians of potential candidates; inform Quality Improvement Committee on program use and any roadblocks.
- Lead and direct special projects as needed.
WHAT WE ARE LOOKING FOR.
- Bachelor's Degree in Social Work or a related degree required.
- Certification as a Social Worker in the State of Michigan required.
- Experience in an acute care hospital or healthcare agency preferred.
- Knowledge and understanding of community health, welfare, and social agencies.
- Understanding of medical terminology.
- Exceptional customer service and communication skills with a professional demeanor that ensures trust, competence and compassion amongst patients and team members.
- Ability to organize and prioritize multiple tasks simultaneously.
- Ability to successfully pass a pre-hire drug screening and background check.
