Job Description
The primary purpose of this position is to perform and oversee client personal care or to assist clients in performing personal care, including meal/cooking/housekeeping services, as designated in client Care Plan. This role is part time, evening shift, weekends and/or morning shifts.
Specific Duties and Responsibilities
Policies and Procedures
Maintain confidentiality of clients, client family members, coworkers, and Immanuel Living
Understand and support the philosophy, policies, and standards of Immanuel Living and the Immanuel at Home Department
Maintain quality of client care, including compliance with client Care Plan
Maintains appropriate boundaries with HomeCare clients, and does not perform duties outside of the designated Care Plan or scheduled time frame for assignment.
Position Duties
Accurately and safely perform client care, in accordance with clients’ Care Plan, client Flow Sheets, and Director (or designee’s) instructions
Follow Client Care Plans and give input for changes in writing
Perform other duties as assigned
Documentation and Communication
Accurately record client care and tasks in SmartCare
Practice appropriate phone etiquette
Communicate with supervisor and participate in training and/or meetings that help facilitate departmental communication process
Attend and participate in in-service educational classes, on-the-job training programs, etc., as scheduled or required. Attend staff and departmental meetings, as directed
Communicate with others in a considerate manner
Other Duties and Responsibilities
Function effectively as a team member
Maintain understanding, patience, and tact in dealing with clients, client families, and coworkers, and be cooperative
Maintain good working relationships with other staff
Respect clients’ personal items
Ensure maintenance of a safe living and working environment for clients and staff
Work independently and maintain good problem-solving skills
Maintain regular work attendance and follow established work schedule. At times, hours and days of the week must be flexible to meet scheduling requirements.
Requirements
Employee must be at least 18 years of age.
Employee must have a working vehicle, valid drivers license and insurance.
The employee must be able to perform multiple tasks simultaneously and work under time and/or deadline pressures.
Education
Graduation from high school or GED preferred.
Current Montana Certified Nurses Assistant certification (CNA), if certified.
Completed Montana CNA Skills Checklist (CNA), if certified.
Completion of six month and annual CNA Skills Evaluation (CNA), if certified.
