Nurse Practitioner – HELP Clinic

Work hours: PRN (As Needed)

Department Highlights:

The HELP Clinic supports nursing operations for both inpatient and outpatient care throughout the hospital and provides comprehensive assistance to 69 patients referred from various sources. The Nurse Practitioner at the clinic has a multifaceted role that involves advocacy and management of health outcomes. The positive outcomes suggest that the program is effective in helping patients manage their health and prevent complications.

Here’s What You Need

Education/Experience

· Bachelor's Degree Graduate of an accredited nursing program and master’s degree Graduate of a Nurse Practitioner program Required

·

2 years’ experience as a nurse practitioner preferred.

·

Experience with community engagement is a plus.

Licenses and Certifications

· RN - Registered Nurse Upon Hire Required

· APRN – Advanced Practice Nurse required

· NP – Nurse Practitioner required

· BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required

· CPI - Crisis Prevention Intervention Training Maintained Annually 90 Days Required

· ACLS - Advanced Cardiac Life Support 30 Days after hire preferred

What You Will Do

Assessment/ Plan of Care/ Technical Skill:

· Utilizes principles and skills in communication and interviewing techniques in obtaining a comprehensive health history to formulate a plan of care and make decisions for appropriate therapeutic interventions

· Uses principles and skills in advanced health assessment for differentiating normal and abnormal findings

· Synthesizes knowledge of physiology, pathophysiology, culture, and socioeconomics to access, evaluate, and provide therapeutic interventions to patients

· Collaborates with the multidisciplinary team to improve care processes in order to promote continuity of care

· Initiates actions to meet patients/families needs

· Takes actions to reduce patients risks

· Promotes population health through the evaluation of care delivery models as they pertain to community, environmental, cultural and socio-economical dimensions of health. Evaluates the effectiveness of interventions

Partnerships:

· Works in partnerships with peers and other disciplines

· Actively participates in formal and informal teaching and evaluation to strengthen clinical competence of self and team members

· Flexible with scheduling to meet the department and patient needs

· Adapts to changes in workload assignments to meet changing needs of the individuals, patient, or the unit

Continuous Quality Improvement:

· Utilizes evidenced based practice

· Demonstrates contribution to unit based programs and research strategies to monitor and continuously improve the quality of patient care

· Assist in the identification of ineffective and costly processes and take actions to improve those processes
