This shift is full time with some 3pm-11:30pm and some 11pm-7:30am.
Responsibilities:
Establishing personal rapport with actual and potential residents and other persons in a position to understand service requirements.
Coordinates and evaluates care of resident in the Personal Care Unit on her shift
Reviewing resident's care plans in accordance with the physician's orders and plan of care
Initiating, coordinating and enforcing policies and procedures
Reviewing work scheduled for nursing personnel on unit on a daily basis to protect needs of the incoming shift
Assigning residents to nursing teams/primary caregivers in collaboration with nurse manager, according to the needs of the residents and the team members' ability using the plan of care as a guide for each individual resident's care
Maintaining staffing levels to follow budgetary guidelines and reporting needs to Nurse Manager
Assisting in writing and updating policies and procedures
Enforcing adherence to State Board of Nursing and Nurse Practice Act requirements and to other governing agency regulations
Measure health outcomes against regulatory and departmental standards
Interpreting care to residents, their families, nursing staff, and other personnel as needed
Giving nursing care to resident who require specialized nursing judgements or skill
Assuring proper medical treatment and care through first hand knowledge of the resident
Assuring accurate communication concerning each resident's care/activities during daily report while maintaining confidentiality
Reviewing critical incidents and discusses them with Nurse Manager and Director of Nursing
Interpreting necessary information to the appropriate staff and at change of shift reports
Accompanying physician on medical rounds and determining when physician needs to be contacted
Responding to emergency calls from the community when Resident Care Department is closed and Westmoreland RN Supervisor requests
Implementing monitoring and documenting restorative programs to prevent decline of residents well being
Practicing and interpreting infection control policy and procedure
Enforcing medication administration, storage procedures and controlled substance regulations
Identifying and reporting problems with supplies, equipment, and physical condition of unit
Participating in Quality Assurance process
Assuring completion of necessary documentation, as well as checking for accuracy and consistency
Completing appropriate reports that includes incident reports, pharmacy consultant report, medical records report, and personal care assessment forms in a timely fashion
Counseling and discipling employees
Assuring the timely completion of evaluations of staff on all shifts
Evaluating need for new equipment and techniques, and reporting needs to appropriate person
Developing budget projections for capital equipment/projects/small equipment
Qualifications:
- Graduation from an accredited school of nursing and current licensure by the Commonwealth of Pennsylvania as an LPN
- A minimum of one year of experience as an LPN where management skill competence has been demonstrated. At least two years of long-term xperience required.
- CPR Certification
- Demonstrates ability to coordinate, supervise RN's, LPN's, and NA's, and secretarial staff, maintain an even temperament in demanding situations, and display leadership, especially during emergency situations
