West Virginia Employer Supported Living Companion in Lewisburg, West Virginia
Assisting individuals w/disabilities in their home & community. Driver's license required as may transport clients as needed. Must have reliable transportation and good driving record. Must pass background check. Will work any of the seven days of the week. Shift work is required. Insurance available, employer will discuss. Pay is 12.50/hr. Does not required a minimum education. Integrate with Community. Other duties as assigned. Will attend and participate in the development of IPP - Advocate for the individuals care, safety, and welfare - Provide active treatment while implementing habilitation goals and objectives - Document all activities of habilitation via a progress note & data sheet/Submit all material on agency approved deadlines - Recognize & report changes in behavior - Attend & participate in regular staff training - Maintain confidentiality - Respect the rights of the individual nd their family - Provide daily documentation of all agency required material - Complete "Critical Incident Report" forms - Complete "Staff Incident Report" forms - Understand & implement the agency's philosophy & mission - Understand & follow Chain of Communication - Work cooperatively with other team members - Serve as a bridge between agency and community - Ensure the safety and well-being of individuals - Shred confidential info. as directed - Implement Positive Behavior Support plans. Must be able to move & lift objects in excess of 100 pounds.
