Located in beautiful Louisville, CO, our 104-bed facility provides a peaceful setting for those seeking treatment for mental illness including chemical dependency. Centennial Peaks offers unique and individualized programming for adolescents and adults that sets us apart from many other treatment facilities. Our hospital is fortunate to provide Electroconvulsive Therapy, Intensive Outpatient and Partial Hospitalization Programs in addition to inpatient services.

Centennial Peaks Hospital is seeking a Patient Access Coordinator to join our compassionate and dedicated Intake team. The Patient Access Coordinator is responsible for managing incoming calls for the Intake Department, reviews all transfer requests, schedule patient assessments and admissions, update and coordinate Patient Flow and Bed Boards, and thorough communications with department RN's and the admissions team to ensure smooth transition into the admission process.The Coordinator also directs expectations for patients regarding their services and responds to customers' inquiries.

Job Duties/Responsibilities:

  • Manage Intake calls.

  • Process transfer requests.

  • Facilitate referrals to outpatient services.

  • Coordinates and processes patient assessments.

  • Maintain strong communication within the team and departments.

    * Must live in Colorado! All training and initial time in the position will be in-person.

    *Must have experience with Behavioral Health. This is a fully remote position!

    The schedule is 3-10hr shifts, 6p - 4:30a, Friday, Saturday, and Sunday

    This position is eligible for our generous shift differentials: Weekend Day (Sat-Sun) $2/hr, Evening (Mon-Thurs) $3.25/hr, Weekend Evening (Fri-Sun) $5.00/hr, Night (Mon-Thurs) $5.00/hr, Weekend Night (Fri-Sun) $7.00/hr. *must work three and a half hours (3.5 hours) into a shift to qualify for shift differential.

    Qualifications

    Requirements:

    * Must live in Colorado! All training and initial time in the position will be in-person.

    *Must have experience with Behavioral Health.

  • The ability to multi-task and thrive in a fast-paced environment is critical.

  • Staff must demonstrate knowledge and skill in provision of services to various populations including children, adolescents, adults, older adults and dually diagnosed patients in an inpatient and partial hospital setting.

  • Bachelor’s degree from an accredited college or university in social work, psychology, mental health, or a related field.

  • A minimum of one year of experience in a human service, social service, psychiatric, or mental health setting, performing administrative duties, crisis intervention, telephonic triage and inquiries, and customer service.

  • Prefer knowledge of crisis and behavior management, safety and security, radio and telephonic switchboard operation, computer systems, and faxing/copying.
