Family Service Association Care Coordinator in Mashpee, Massachusetts

We are offering a \$1,000 sign on bonus! We have a full time (40 hours per week) The hours are Monday through Friday, 8:00 a.m. - 5:00 p.m. Care Coordinator position in our Long-Term Services & Supports (LTSS) Community Partner (CP) program in the Cape Cod region. This position is partially remote with 2 drop-in days in Mashpee office in Cape Cod area, & includes travel to Outer/Lower Cape Cod with occasional travel to the Fall River corporate office. The Care Coordinator is responsible for promoting coordination in the delivery & receipt of LTSS to enrollees assigned by Accountable Care Organizations (ACO) & Managed Care Organizations (MCO). Responsibilities include: Conduct outreach to assigned enrollees to inform about the functions of a Care Coordinator, the optional nature of enrollment, choice in CP providers, the benefits of enrollment & the process for enrolling. Document all outreach attempts including 3 separate attempts; & at least one face-to-face attempt unless the enrollee voices a preference for only telephonic contact. Execute all required documents for participation in the CP program including but not limited to a Participation Form & all required authorizations for Use & Disclosure. Obtain the Comprehensive Assessment from the referring ACO/MCO & update as needed for any changes in functional status, ADL or IADL needs. Utilizing the Comprehensive Assessment, & under the direction of the enrollee/authorized representative, develop a LTSS Care Plan that identifies preferences, goals, strengths, needs & cultural considerations & is signed by the enrollee/authorized representative. Complete updates to the LTSS Care Plan upon request of the enrollee, as needed due to major changes in functional, ADL or IADL status, & at least every 12 months Ensure LTSS Care Plan is approved by the Primary Care Provider & shared with all members of the enrollees care team including providers, members of state agencies & other case managers within 90 days of assignment. Participate as a member of the enrollees care team, providing subject matter expertise to the care team about LTSS, the enrollee?s needs, preferences & service options; advocating for appropriate care, facilitating communication with other providers & promoting integration of care across all spectrums. Maintain regular contact with the enrollee & a minimum of one face-to-face contact quarterly to assist the enrollee to navigate & access needed services, providing ongoing monitoring of the implementation of the LTSS Care Plan & monitoring for changes in status including health, housing, natural supports & caregivers. Maintain all documentation required for the delivery of LTSS Community Partner services. Foster & maintain collaborative relationships with Primary Care Providers, Social Service Providers & representatives from state agencies. Experience & Educational Requirements: Must have a minimum of a High School Diploma or Equivalent (GED) A Bachelor\'s Degree in social work, human services, nursing, psychology, sociology, or a related field if preferred. Three years of experience in a related capacity & at least one year of experience in the field. Must have strong written & oral communication skills & demonstrated computer skills. Bilingual a plus & will earn an extra \$1 per hour for that skill. This is a benefits eligible position.
