Baptist Memorial Navigator-Patient ILD in Memphis, Tennessee
Summary
Serves as the primary point of contact for the multi-disciplinary Interstitial Lung Disease (ILD) program. Will be a part of a multi-disciplinary team including pulmonologists, radiologists, pathologists, rheumatologists, respiratory therapists, physical therapists, medical assistant and smoking cessation counselors. As a key member of the team, the Patient Navigator will be responsible for keeping track of our patients, follow up tests, referrals and clinic follow up appointments, and coordinating the monthly multi-disciplinary meeting. The Patient Navigator will build strong relationships with patients and assist with their engagement in medical care and adhering to medications. They will work closely with the patient's care team as well as help find financial aid for medications to support positive outcomes.
Responsibilities
Meet with Care Team (including, but not limited to, Care Coordinator and primary care provider) to discuss patient care issues, needs and facilitate patient health care.
Maintain documentation of all patient encounters and complete reporting requirements according to organization standards.
Track patient information, schedules, files, and forms in a confidential manner.
Track patient attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.
Requirements, Preferences and Experience
Experience
Minimum
- 3 years healthcare experience.
Licensure
Minimum
- Licensed in a clinical discipline such as RRT, CRT, LPN
Special Skills
Minimum
Proficient in data entry
Proficient in disease state
Knowledge of the approval, initiation and maintenance of interstitial lung therapies.
Must have interpersonal skills sufficient to effectively and cordially interact with patients, families, staff, medical staff and visitors.
Must have time management, analytical and negotiation skills.
