Responsibilities

Function as a liaison between internal/external referral sources and inpatient rehabilitation

operations.

  • Establish relationships and aid with communication to case managers, physicians, payors, patients, families and referral sources in order to generate referrals and drive patient admission volume in the inpatient rehab.

  • Ensure appropriate patient selection through clinical chart assessments during on/off-site clinicalevaluation of patients in collaboration with the physician.

  • Complete in-service and educational opportunities for physicians, case managers, interdisciplinary members and other professionals. Serves as an educational resource to patients, family members and caregivers.

  • Participate in the development and implementation of business plans for the rehab unit and contributes to the development and maintenance of referral source promotional information.

Requirements

Licensure / Certification / Registration:

  • One or more of the following required:

  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.

  • Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.

  • Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.

  • Speech-Language Pathologist credentialed from the Wisconsin Hearing and Speech Examining Board obtained prior to hire date or job transfer date.

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

  • Drivers License preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences

Previous experience preferred. Excellent organizational and multitasking skills. Ability to assess a patient's clinical status and medical necessity to determine appropriateness for admission to an inpatient rehabilitation setting. Confident with medical terminology and regulations to be admitted to an inpatient rehab unit. Excellent communication with medical team, patients and support team. Flexible with assignments, able to travel between hospitals. Able to work comfortably on a computer within the electronic health record and other systems online.
