Elder Care Transition Specialist
ACCS provides wraparound support through an integrated team of both clinical and peer professionals. The goals of the service include skill and resource development, connection with community and natural supports for successful tenure in the community of the individual’s choice.
The Elder Care Transition Specialist will provide direct assessment of individuals enrolled in ACCS, as well as consultation and support to the team. The Elder Care Specialist will assist clinicians in assessment, identifying appropriate level of care (LOC), developing plans for support and transition to nursing, and rehab facilities, as well as senior living or rest home.
Responsibilities include:
Conduct assessments in collaboration with team Nurse, Occupational Therapist, Clinician as needed and assist in establishing treatment plans.
Consult with the team on necessary elements to successful ‘aging in place’
Build collaborative relationships with area nursing facilities, rest homes, senior services, etc.
Provide consultation, training, education and resources to families, treatment team, and ACCS staff.
Establish and maintain collaborative relationship with DMH Nurse Manager
Develop a through understating of PASSR regulations and requirements
Schedule: Full Time, 40 hours
Pay Rate: $25.00/hourly

Required Skills
Must have a valid driver’s license and regular access to a reliable vehicle.
Must be able to work as a member of a team and to function independently.
Required Experience
Bachelor’s Degree required
At least three years working within elder care either in the community or in a facility required.
Degree in one of the following preferred: nursing, occupational therapy, social work
