POSITION SUMMARY:

Under the direction of a Registered Nurse (RN), The Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes with patient & family satisfaction. He/she operates within the scope of practice defined by the State Nurse Practice Act. The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided by unlicensed staff.

RESPONSIBILITIES:

  • Makes daily compliance rounds on assigned unit to observe, examine and/or interview residents, verify information from ongoing shift, monitor regulatory compliance, determine staff assignments, and provides/ receives report.
  • Provides orientation on unit and unit routines to new staff members or staff from temporary agencies.
  • Develops and distributes resident care assignments to direct care staff.
  • Describes care plan goals and approaches to direct care staff to ensure the care plan is consistently implemented on all shifts by all staff. •Monitor the delivery of care and services throughout the entire shift. This ensures the residents needs are met, tasks are completed, and direct care staff have provided quality work. Administers medications, treatments, and provides care according to physician orders while maintaining facility compliance.
  • Performs physical assessment (or gathers appropriate data) of new admissions and current residents as indicated by change in condition or as required by regulation. Completes assigned sections of resident assessment instruments used in facility.
  • Work hand & hand with the interdisciplinary team to ensure the resident receives optimal care while ensuring the plan of care is adhered to.
  • Modify approaches in the residents plan of care to accommodate residents needs and preferences, to avoid/manage risk factors for functional decline or to improve functional abilities.
  • Respects "Residents Rights" regarding freedom of choice, consent for care and services, refusal of treatment and implementation of advance directives if any. Promptly consults with Nursing Supervisor, Unit Manager, and/or Social Worker when confronted with a "Residents Rights" issue.

Qualifications

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:

    1. Graduate of an approved practical/vocational nursing school
    1. Active license in the state of Pennsylvania, CPR Certified
    1. Long Term Care experience preferred
