Intake Coordinator
The Opportunity
FCA is excited to announce an opportunity for the position of Intake Coordinator for our EISEP Program. The EISEP Intake Coordinator oversees and coordinates the intake process and coordination of services for the EISEP and SAFE programs.
Responsibilities
Provide an initial evaluation of the senior adult's needs and prioritize the cases that come into the program.
Follow up with all cases until assigned to a case manager.
Provide information and referral to clients as requested.
Provide case management coverage in absence of the case manager, handle emergency cases and walk-ins.
Prepare and maintain client case records in the data base (Peer Place) and prepare and keep statistics as required by the standards set by OFA and FCA.
Conduct assessment for SAFE eligibility and refer for emergency fuel.
Assume other tasks and responsibilities as assigned.
Qualifications
BSW or related degree required.
Experience with geriatric population preferred.
Strong verbal and written communication skills.
Computer proficiency with MS office required.
Valid and clean New York State Driver's License
Additional Information
Salary or Hourly: $29.18/hr
Schedule: Monday-Friday, 9am-5pm
