Minnesota Visiting Nurse Agency Mental Health Professional - Licensed – HOPE Program for First Episode Psychosis (hiring incentive) in Minneapolis, Minnesota

We are currently seeking aMental Health Professional – Licensedto join ourFirst Episode Psychosis (FEP) team.This full-time role will be on site Monday-Friday. This role is part ofan interdisciplinary team for early interventions for schizophrenia.This is a grant funded role, currently funded through 6/30/2025.

Purpose of this position: Following an empirically-supported treatment model, a mental health professional provides direct mental health services including screening, diagnostic assessment, psychoeducation, and psychotherapy services to persons living with mental illness; evaluates care and programming; and develops programming as needed.

Work in a multidisciplinary (MD, LP, MHP, RN, case management, voc rehab, peer support) clinic offering psychotherapy. You will experience diversity in your practice while offering diagnostic assessments, treatment planning, facilitation of groups and individual sessions to patients. A $5000.00 hiring incentive is available for eligible external candidates (contract/temporary workers, current employees, and former employees returning to HHS within one year are ineligible).

RESPONSIBILITIES:

  • Participates in multidisciplinary care team rounds
  • Provides mental health services including initial and ongoing screening, diagnostic assessment, treatment planning (including regular updates), psychoeducation, psychotherapy services, crisis services, referrals, and discharge/termination planning
  • Completes necessary documentation for accurate and timely billing of clinical services provided
  • Provides psychoeducation to patients, families, and community organizations as needed
  • Provides care coordination including discharge/termination planning
  • Assists in maintaining standards of accreditation by health management organizations and state licensure/certification
  • Other duties as assigned

QUALIFICATIONS: /**/

/Minimum Qualifications:/

  • Meets criteria for “Mental Health Professional” as defined in MN statute
  • Master’s degree in a behavioral health field

/Preferred Qualifications:/

  • Experienceproviding psychotherapy groups and experience working with adolescents and young adult persons living with mental illness, especially psychotic disorders, and addiction

/Knowledge/ Skills/ Abilities:/

  • Ability to counsel and instruct patients and families
  • Ability to recognize and document patient progress
  • Ability to work cooperatively with other members of the multidisciplinary clinical team
  • Ability to interact effectively with individuals from diverse backgrounds including ability to deal objectively with individuals whose attitudes and behaviors differ from her/his own
  • Ability to read and interpret written instructions, patient charts, and physicians' notes
  • Ability to interpret policies and procedures in an advisory capacity
  • Ability to use computer systems, including electronic medical records, along with working knowledge of Microsoft Office Products (Outlook, etc.)

/License/Certifications:/

  • Must be a licensed Mental Health Professional per MN statute – LMFT, LICSW, or LPCC
