Position Description
The Division of Prevention and Wellness advances policies and practices that promote healthy behaviors and mitigate the harm (e.g., early death) of chronic diseases. Our division is hiring a part-time grant-funded position that supports one-on-one tobacco (smoking and vaping) cessation with pregnant and recently delivered families with infants up to 12 months of age. This role will also contribute to Prevention and Wellness strategies that strive to create social and physical environments that promote good health and achieve health equity in Davidson County and beyond. This is a self-directed and independent non-supervisory position that reports to the Prevention and Wellness Division Director.

This position will report to the office and work in the community as directed.
Typical Duties
Uses motivational interviewing techniques with participants to support cessation goals
Maintains a monthly participant cessation case load.
Uses nursing or relevant field expertise to work collaboratively with Prevention and Wellness teams and
other public health programs to develop relationships with Obstetrical providers and Managed Care
Organizations to identify early-stage pregnant women who could benefit from cessation support.
Completes monthly program monitoring report
Coordinates billing data with Finance
Creates quarterly progress report and other relevant reports.
Works with data analysts and others to identify successful program operations, analyzing program data.
Works collaboratively with the Public Information Officer to develop a monthly social media calendar of
related content for tobacco cessation support.
Provides one-on-one tobacco cessation support with pregnant and recently delivered families with infants
up to 12 months of age using the “Growing Inside Free of Tobacco and Smoke” (G.I.F.T.S.) Program.
Receives and documents referrals for individuals and families needing smoking cessation support
Schedules initial assessment appointments with referred individuals
Assesses tobacco use (vape and combustion use) and develops cessation plan
Objectively monitors participants tobacco consumption via saliva samples and carbon dioxide monitoring at
regular intervals in accordance with cessation plan
Maintains electronic case notes on participant engagements using the SOAP note method (Subjective,
Objective, Assessment, Plan)
May refer participant to other identified services as needed and with participant’s consent

Promotes the benefits of tobacco cessation during and after pregnancy and through the infant’s first year of life.
Minimum Qualifications
Diploma or Associate’s Degree in nursing. Candidates with Diploma or Associate’s Degree in nursing must establish a documented ability to competently perform all functions of this classification to be considered. Bachelor’s degree from a state-approved school of nursing preferred.
Licensed as a Registered Nurse in the State of Tennessee. Applicants holding a Registered Nurse license from a state that participates in the “Enhanced Nurse Licensure Compact” may be hired, but must obtain their Tennessee license within thirty (30) days of hire.
Cardiopulmonary Resuscitation (CPR) Certification.
All public health nurses are required to complete a department approved Certificate in Public Health as a condition of continued employment. Possess required competencies upon hiring.
Valid class “D” driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.

Preferred Experience, Knowledge, Skills, and Abilities
Knowledge of policy, systems, and behavioral change strategies to address tobacco cessation.
Experience in health education and program implementation.
Demonstrated ability to engage with diverse community stakeholders to complete tasks and achieve goals.
Previous experience in obstetrical nursing or perinatal case management capacity.
