Position Overview:
The Incident Manager reports to the Chief Housing & Shelter Officer and maintains oversight of incident management, investigation, and review processes for Project Renewal. This position assumes a key role to ensure adherence to policies and promote a culture of learning and reflection from incident response and review. This person serves as a liaison between the department, programs, and will work closely with the Chief Compliance and Privacy Officer around Justice Center Investigations.
Essential Duties & Responsibilities:
Manage and lead internal systems for incident reporting, investigation, review, and incident data tracking
Review all incidents reported in the agency’s internal database for completeness, clarity, and adherence to established incident documentation standards
Compile reports of all incidents and identify which require investigation
Facilitate PRI’s internal agency incident review meetings (at least 20 meetings a year), covering OMH/OASAS-licensed program incidents, shelter program incidents, all other agency program incidents, and mortality reviews
Record and disseminate Incident Review Committee (IRC) minutes and recommendations to program staff
Collaborate with programs and senior leadership to develop and manage corrective action plans
Track documentation related to client deaths, coordinating with program staff and healthcare leadership to ensure timely and complete review of contributing factors
Serve as primary investigator and agency liaison with the NYS Justice Center for the Protection of People with Special Needs
Coordinate with program staff at licensed facilities to ensure that all reportable incidents are reported to the Justice Center, OASAS, and/or OMH within required timeframes
Submit investigative reports and supporting documentation to external reporting systems and agencies, such as Justice Center’s Administrative Action Reporting Mechanism (AARM), Web Submission Investigation Report (WSIR), OMH NIMRS and to OASAS and other agencies and systems as identified
Coordinate agency response to concerns and compliance reported through OMH Field Office
Partner with data management team to generate and disseminate analyses of key incident trends and maintain updated tracker of Justice Center Incidents and Client Deaths
Revise incident management policies and procedures as appropriate and collaborate with the Chief Compliance and Privacy Officer in revising such policies
Conduct regular trainings in person and virtually on incident management and incident reporting topics, including, but not limited to: Understanding Incident Reporting Guidelines, Understanding Classifications of Incidents, and Best Practices Writing Incidents
Qualifications:
Bachelor’s degree required, Master’s degree with a major in Social Work, Public Administration, Psychology, Sociology, or other Human Services Field preferred
Minimum 3 years’ experience working in the social service field, with familiarity with DHS, OMH, DOHMH, and/or HASA, a plus
Excellent computer skills, including Outlook, Microsoft Word, Excel, PowerPoint, and Microsoft Teams and Zoom
Familiarity with AWARDS, DHS CARES, and NIMRS a plus
Physical Activities and Environment: While performing the duties of this position, the employee may work in an office environment, travel to PRI facilities or meetings/events outside PRI offices and facilities. T he employee is required to talk and hear, often required to sit for extended periods of time.
All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
