JOB PURPOSE
Maintain a caseload of older adult clients; assessment of client's core needs, provision of case management and supportive counselling services.
SCOPE OF INFLUENCE
Assist clients with a range of complex issues related to aging.
Facilitate client-led goal setting
Provide advocacy to clients in navigating challenging life issues.
KEY RESPONSIBILITIES AND ACCOUNTABILITIES
Carry a caseload of older adult clients, providing comprehensive assessment of needs, case management, supportive counselling and routine follow-up.
Formulate care plans and ongoing monitoring of clients.
Maintain up-to-date case records and program statistics in Peer Place
Assist with organizing NORC events as needed.
Participate in biweekly case conference
Work with an interdisciplinary team to ensure consistency of service to clients.
Coordinate with external agencies to ensure best care for client.
Attend supervisory, agency and community meetings as required.
Perform other related duties as assigned by supervisor.
JOB REQUIREMENTS
LMSW or equivalent.
Experience and enthusiasm for working with older adults.
Knowledge of resources and entitlements, as well as knowledge of age-related cognitive and mental impairments required.
Attention to detail, ability to work under pressure, and effectively coordinate a comprehensive array of services to address the multiple needs of the NORC population.
Ability to take initiative and work independently.
Ability to maintain confidentiality of records and information. Must have outstanding verbal and written communication skills.
Willingness to work as part of a team.
Knowledge of MS Office preferred.
