East Bay Community Action Program Community Health Worker (CHW)- Transgender Whole Healthcare Program in Newport, Rhode Island

East Bay Community Action Program (EBCAP) is seeking a full time Community Health Worker for our Transgender Whole Healthcare Program at our Newport location. The Community Health Worker will collaborate with members of a multidisciplinary community health team comprised of community health staff, behavioral health master level clinicians and primary care practice staff to promote patient-centered care in support of various programs. Provide peer navigation to any LGBTQIA+ eligible patients, with the goal of increased independence accessing services and increased whole-person health. The CHW will also have opportunities to facilitate educational seminars on LGBTQIA+ Healthcare, advocate for the needs of the LGBTQIA+ community, and more.

The Community Health Worker requires a minimum of an Associates Degree in a social science, research or public health - related field preferred. and or 10 plus years of experience working in behavioral health. Must have either lived or professional experience navigating the landscape of gender affirming healthcare in Rhode Island. Certification through the RI Certification Board (RICB) as a Community Health Worker or requirements met to sit for CHW certification exam within 12 months. (Requirements include: six months or 1000 hours of paid or volunteer work experience within five years, 50 hours of supervision, 70 hours of education relevant to the domains established by the RICB, portfolio as designated by the RICB, and recertification and continuation education every two years.)
