Liberty Home Care RN Field Supervisor in Niagara Falls, New York
Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team! This position would focus primarily on Niagara County and Northern Erie County service areas and offers either a traditional Monday - Friday 9a-5p or a non-traditional schedule that can be tailored to fit your needs.
Duties & Responsibilities
Provide assessment of newly referred patients and assess health needs in home care setting.
Work with patient to create a patient centered plan of care.
Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
Provide supervision and training of PCA/HHA staff.
Assist with the on-call rotation (stipend provided)
Qualifications
NYS Licensed Registered Professional Nurse.
Minimum of Associate's Degree required.
One year Nursing experience required.
Home care experience a plus.
