Summary This Social Worker position serves as the Community Residential Care Program Coordinator (CRC) for the CRC Program at the Central Arkansas Veterans Healthcare System (CAHVS) and is responsible for overall program development, management, operations and evaluation of the CRC Program. Responsibilities Duties of the Social Worker (Program Coordinator) include, but are not limited to: Providing the appropriate level of care and an improved quality of life for Veterans who do not require hospital or a skilled nursing facility, but who are capable of independent living; Facilitating the most appropriate use of VA and community resources; Maintaining or improving the Veteran's health and social functioning in a supportive environment; Supporting the highest level of functioning of the Veteran including discharge to independent living, when possible; Providing a home environment where the Veteran may remain in comfort, retain dignity, and have the needed support through the end of life, guided by Veteran preference and feasibility; Recruiting and screening CRC facilities; Overseeing Veteran referral and monitoring placement into CRC facilities planning and developing of CRC sponsor educational programming; Record keeping in compliance with Federal regulations, state licensing requirements, and local regulations; Maintains records for each CRC home including but not limited to: facility license, inspections, background checks and education information; Coordinating of VA inspections of CRCs Monitoring and reporting quality of the CRC and evaluating status of the Veterans residing in VA approved CRC homes; Ensuring that all deficiencies are corrected, establishing timelines or deadlines for correcting of any deficiencies; Visiting each CRC facility monthly for oversight and compliance with standards; Maintains CRC database for tracking purposes, which includes but not limited to room and board rates, sponsors and persons served information, discharges, consults, etc.; Completes CRC quarterly reports and submits to national CRC director timely; Coordinates the day-to-day CRC placements, monitors quality of care received in the CRC, adverse events in CRC facilities, analyzes data gathered and reports to the Community Residential Care Oversight Committee, Director of Geriatrics & Long Term Care and the Medical Center Director significant problems and concerns; Responsible for taking appropriate actions required and follow all standard operating procedures regarding investigation of sentinel/adverse events within the program; Establishes processes and collaborates with VA HUD-VASH and local PHA's for the use of Housing Choice Vouchers (HCV) that are allocated by the Department of Housing and Urban Development for Veterans experiencing homelessness; Promotes and markets the HUD-VASH/GEC initiative and CRC services. Driver's License Required: This position requires a current, valid driver's license to drive a government vehicle on a regular and recurring basis to perform the duties of this position. The incumbent is expected to drive within the catchment area of CAVHS to supervise approved VA Community Residential Care homes, provide support to CRC sponsors and Veterans and conduct outreach efforts. This means there will be checks of driving records for approval based on driving history including motor vehicle accidents, speeding, etc. Work Schedule: Monday-Friday, 8:00am-4:30pm Telework: Not Available Virtual: This is not a virtual position. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience Licensure/Certification: Individuals assigned as Social Worker (Program Coordinator) must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure (except for licenses issued in California, which administers its own clinical examination for advanced practice). Demonstrated Knowledge, Skills, and Abilities (KSAs): In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Two years working with individuals with serious mental illness Performs mental health evaluations for a diverse population, including geriatric and post-deployment combat Veterans, who have a serious mentally illness, such as schizophrenia, schizoaffective, and bipolar disorders and who may also have psychosocial issues such as homelessness, unemployment, or a poor family support network. Two years experience collaborating with community stakeholders Ensures practice of mental healthcare is consistent with TJC, HIPPA, VA policies and procedures, clinical practice guidelines, national statutes and regulations, medical record documentation, and maintain mandatory continuing education. Maintains appropriate licensing and board certifications as applicable. Participates effectively in team meetings and collaborates with interdisciplinary team members to enhance coordination of comprehensive patient care. Orders consultations appropriately. Discriminates between normal and abnormal findings and makes appropriate treatment decisions. Basic Life Support (BLS) Certification.
