Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary This position provides planning, coordination and monitoring of mental health services to adults with serious and persistent mental illnesses who are eligible for Rule 79 or Minnesota Community Healthcare Network case management services. It is collaborative in nature and requires working closely with Community Services, Court Services, private providers, outside resources, Healthcare organizations, and programs at Canvas Health. Location Oakdale Schedule Monday through Friday Minimum Qualifications Bachelor's degree or higher in the human services or social work field 2,000 hours of supervised experience in the delivery of clinical services in the treatment of emotionally disturbed children and/or adolescents. Two years' experience preferred working with mentally ill and/or chemically dependent individuals, of which at least one year is experience in community-based mental health, chemical health treatment with adults recovering from mental illness/chemical dependency or combination of equivalent experience and training. Preferred Qualifications One year in a case management or similar role Experience working with and/or delivering services to diverse populations. Experience and specialty training in crisis intervention and/or working with SPMI clients within the past two years and knowledge of local social and human service networks Experience with Electronic Health Record systems As an essential healthcare provider, Canvas Health follows all local, state, and federal guidelines regarding COVID-19 precautions.
