Overview
*$7,500 Sign-On Bonus and eligible for up to $10,000 Student Loan Assistance*
As our Social Worker at CHI Health Immanuel, you’ll perform social work assessments and interventions 55+ patients in the Heritage Acute Behavioral Health Unit. Working collaboratively with all healthcare team members, you will develop and implement treatment plans that support patient-centered care for individual patients and the medical community. Being dedicated to patients and family-centered care, you value the personal self-determination of your patients. To succeed in this role, you must manage multiple priorities simultaneously and engage in creative, compassionate, and ethical problem-solving.
Responsibilities
Perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations
Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
Serve on hospital committees and develop and present in-services to Hospital staff.

Qualifications
Required Education for Staff Job Levels
Master’s Degree in Social Work Required
Required Licensure and Certifications
State of Nebraska licensure required prior to start date
